Responsible for diabetes/nutrition education of inpatients and outpatients, their families and appropriate support systems. The diabetes education is focused on diabetes self-management and related skills, The teaching is individualized for persons of all ages, incorporating their cultural preferences, health beliefs and preferred learning styles, when feasible. Incorporates American Diabetes Association, American Dietetic Association and American Heart Association standards.
Responsibilities:
A day in the life of a Clinical Nutrician Educator at Hackensack Meridian Health includes:
Assesses diabetes management and nutritional needs of each patient and/or their families.
Plans education based on patient needs, age, culture, ethnicity, and education level.
Implements education for each patient.
Evaluates patient understanding of education.
Incorporates lab values, nutritional needs, and meal planning standards of American Diabetes Association, American Dietetic Association, and American Heart Association into education.
Plans, implements, and evaluates diabetes and nutrition education for the healthcare team and general public.
Mentors professional students.
Participates on interdisciplinary teams and/or hospital or community committees to act as a resource in diabetes management or nutrition.
Communicates updates on diabetes management and healthy nutrition to the community by participating in health fairs, performing diabetes screening for community groups and middle school children, and lecturing to community groups.
Collaborates with physician and healthcare team to implement and evaluate the patient's plan of care.
Informs physicians of patient's progress toward goal on the appropriate documentation form. Informs physician of patient issues in a timely fashion.
Obtains prescriptions/referrals as necessary. Assists in the registration process.
Develops education materials that meet American Diabetes Association, American Dietetic Association, and American Heart Association standards. Develops documentation forms to meet hospital and certification agency requirements.
Develops and implements patient teaching aids.
Documents all sessions with each patient according to policies and procedures. Documents patient progress toward goals on appropriate form.
Records charges based on time spent with the patient. Follows department guidelines for productivity. Assists in Diabetes Center efforts to increase volume.
Other duties and/or projects as assigned.
Adheres to HMH Organizational competencies and standards of behavior.
Qualifications:
Education, Knowledge, Skills and Abilities Required:
Registered dietitian with BA in food and nutrition
Minimum of 3 years in health education or health related experience.
Two years in leadership, managerial or education capacity preferred with a strong background in diabetes education.
Knowledge of teaching principles; strong organizational skills, knowledge of state and federal reimbursement for outpatient diabetes education; presentation skills
computer skills and computer application knowledge
Education, Knowledge, Skills and Abilities Preferred:
Insulin Pump Trainer.
Licenses and Certifications Required:
Registered Dietician License.
Certified Diabetes Educator.
