Job description

Job Purpose:

The LPN functions under the supervision and guidance of the Nurse Manager, RN Case Manager, RN Team Lead, and/or Provider(s). Participates in the
multi-disciplinary process of providing care and treatment to patients. They provide clinical support and technical assistance to the professional medical and other staff. The LPN is responsible for functioning within the scope of practice dictated by the authority of the Louisiana State Board of Practical Nurse Examiners. Individual must be able to manage demanding workload with accuracy. Position requires excellent customer service skills with patients and their families.

Essential job functions including, but not limited to:

1. Satisfies Core Competencies
• Accepts responsibility for ensuring that his/her nursing practice and conduct meet the standards of the professional, ethical and relevant legislated requirements

2. Patient Relations:
• Achieves customer/patient's satisfaction by greeting and proactively attending to patient needs.
• Functions as case manager and advocate for providers' patient population, assisting the patient in navigating the healthcare field.
• Responds to patient's requests in a timely and efficient manner.
• Assists in making patient appointments and obtain all necessary patient information
• Maintains confidentiality of patient's information at work and off duty.
• Relays messages in a timely manner to appropriate health care providers.

3. Clinical:
• Assists Providers with patient office visits which include rooming patients, interviewing patients, and entering information in patient electronic medical record.
• Obtains and records vital signs, recognize any variances, outliers or red flags, and takes appropriate action
• Administers and documents medication under provider's order and follows the six (6) medication rights and reduces the potential for medical error
• Performs point-of-care lab tests; obtain and process specimens for outside laboratories
• Plans nursing care following agency policies, standard operating procedures, and standing orders when appropriate.
• Compiles and assists providers in responses to triages from team members and patients received by phone or electronic messages

4. Patient Education:
• Provides education related to preventive health topics such as tobacco cessation, nutrition, and physical activity, immunizations, recommended health screenings, sexually transmitted infections (STI's), etc.
• Provides education related to medication adherence, appointment compliance, and general health
• Provider's pediatric education related to child safety, developmental milestones, feeding, oral health, etc.

5. Clerical:
• Maintains patient charts as outlined by each practitioner and according to policy of the Health Center.
• Ensures that an efficient system of communication and processing medical calls.
• Ensures that appointment confirmations are done on a daily basis.
• Participates in morning huddle as assigned
• Orders and retrieves patient's medical records, x-ray films, scans, lab work, etc. in preparation for the patient's office visit.
• Assist in making patient appointments and placing necessary referrals
• Performs administrative tasks, including form completions, prior authorizations, prescription refills and referrals.

6. Safety/Infection Control:
• Strictly adheres to Universal Precautions
• Performs aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors.
• Follows job safety requirements. Reports and documents accidents or incidents within 24
hours of occurrence to the immediate supervisor.
• Organizes exam rooms, including general cleaning, upkeep, and re-stocking of all supplies

7. Workforce Development:
• Responsible for maintaining up-to-date knowledge, skills, and abilities

o Attends and participates in in-service, monthly staff meetings, continuing education offerings, community training events, conferences and
webinars as appropriate

8. Testing, Counseling and Referrals

  • Provide HIV/STD Counseling, Testing and Referrals to at-risk populations utilizing program testing procedures.
  • Provide specialized HIV/STD prevention education and develops individualized risk reduction plan with client.
  • Provides follow-up services for testing recipients in the form of post-test counseling, disclosure of test results and refers as appropriate.
  • Act as a resource to provide clients to other health and social supportive services as needed.
  • Assist individuals in linkage to HIV primary medical care and other support services. Verify that services were accessed post-referral.
  • Communicate effectively with clients for appointments and follow-up on referrals.
  • Distribute condoms to target population and clients receiving testing services.
  • Maintain accurate and current process, procedural, and program, and inventory logs of specific testing activities, health materials, condoms/lube, Temperature log, Test Results, and testing controls.
  • Conduct scientific based prevention education sessions with clients receiving testing services
  • Assist with recruitment for prevention and health services

9.Documentation and Reporting
  • Collect data, conduct data entry and prepare monthly program statistics.
  • Maintain accurate and complete client records, counseling, testing, referrals, and outreach performance as defined by evidence-based models, local/state/federal entities and the department.
  • Enter client data into existing data base systems. Maintains client files.

10. Other Duties
  • Participate in outreach services to target population; this includes preparation and distribution of educational, promotional and recruitment materials.
  • Maintain client confidentiality and comply with all federal and state health information privacy laws.
  • Participate in ongoing quality improvement objectives.
  • Resolve or address client requests within a timely manner.

Required Education:

High school diploma or equivalent

Skill, Licensure and Knowledge Requirements:

Current LA LPN License required

3-5 years of primary care experience in a clinical setting
(preference may be given to those with prior work experience in infectious
disease and/or HIV depending on position)

Current Basic Life Support (BLS) Certification required
