Full - Time: Monday - Friday, No night, weekend or holidays coverage

Position Overview: Oak Medical is seeking an experienced Advanced Practice Provider (NP/PA) with expertise in internal medicine to deliver high-quality, onsite medical care in skilled nursing facilities. This role is focused on providing both acute and chronic care management for adult and geriatric patients. The APP will work collaboratively with supervising physicians to develop and implement comprehensive treatment plans that improve patient outcomes and quality of life.
Key Responsibilities:
Deliver primary care for adult and geriatric patients, addressing both acute and chronic health conditions.
Develop personalized treatment plans in close coordination with the facility care team and other healthcare providers.
Establish medical diagnosis, prescribe medications, perform diagnostic and therapeutic procedures and other care coordination as needed.
Ensure timely, accurate, and thorough documentation of patient assessment and care plans.
Work closely with specialty providers (e.g., wound care, psychiatry) to coordinate integrated care when needed.
Communicate changes in patient conditions and treatment plans to the clinical team.
Communicate with patients, families and staff to support understanding of care plans and interventions.
Foster strong relationships with patients, their families, and the facility staff to enhance care quality.
Qualifications:
Advanced Practice Provider license (NP or PA).
Valid Wisconsin APNP or PA-C License.
Valid DEA.
AHA BLS certified.
Minimum 1year Advance Practice Provider experience (preferred.)
Experience working in skilled nursing facilities or similar settings is preferred.
Strong diagnostic, clinical, and interpersonal skills.
Ability to work independently and collaborate with interdisciplinary teams.
