MDS Registered Nurse

Position Summary:

The MDS Registered Nurse is responsible for completing and coordinating the Minimum Data Set (MDS) assessments to ensure regulatory compliance and accurate resident care planning. This role plays a critical part in the care planning process, working alongside the interdisciplinary team to assess residents' health needs, create individualized care plans, and contribute to optimizing the quality of care provided in the facility. The MDS RN ensures that all assessments are conducted in accordance with federal and state guidelines, specifically in long-term care, assisted living, and memory care settings.

Key Responsibilities:


  • MDS Assessments: Conduct and coordinate MDS assessments, ensuring all sections are completed accurately and in compliance with regulatory guidelines.
  • Resident Assessments: Perform detailed clinical assessments of residents to gather data related to physical, emotional, and cognitive conditions, ensuring that assessments reflect the current status of each resident.
  • Care Plan Development: Collaborate with the interdisciplinary team to develop individualized care plans based on MDS data that meet the unique needs of each resident.
  • Compliance and Documentation: Ensure that all MDS documentation is completed within the required timeframes and meets federal and state regulations, including CMS and OBRA guidelines.
  • Interdisciplinary Communication: Serve as an essential member of the interdisciplinary team, providing input and guidance to ensure that resident care is well-coordinated and appropriate based on assessment data.
  • Data Accuracy: Verify the accuracy and completeness of MDS data, ensuring that all clinical information is correctly documented to support care plans and reimbursement.
  • Quality Improvement: Participate in quality assurance and performance improvement (QAPI) initiatives by using MDS data to help identify areas for clinical improvement and promote better care outcomes.
  • Education and Training: Provide support and education to nursing staff and other members of the healthcare team regarding MDS processes, regulatory updates, and resident care strategies.

Qualifications:


  • Active RN License
  • Minimum of 2 years of nursing experience in long-term care, ideally in senior living, assisted living, or memory care.
  • Familiarity with MDS processes and related regulations, including Medicare, Medicaid, and RAI guidelines.
  • Strong clinical assessment skills with a thorough understanding of the needs of residents in long-term care settings.
  • Attention to detail and ability to manage multiple tasks and meet deadlines.
  • Strong communication skills to effectively work with interdisciplinary teams and communicate with residents and families.

Preferred Qualifications:


  • Certification in MDS or Resident Assessment (RAC-CT) preferred.
  • Experience with electronic health records (EHR) and MDS submission systems.
  • Knowledge of reimbursement models, including PDPM and RUG-IV.
