Successful WSP and FBI background clearance check; 2. Washington State Sexual Misconduct Disclosure Release Form; 3. I-9 Employment Eligibility (citizen or legal alien status); 4. W-4 Form; 5. Washington State Department of Retirement Status Form; 6. Immunization History (for persons born January 1, 1957 or after) - Completion of District Form to be placed on file with the District Nurse; 7. Online mandatory trainings per Valley School District policy; 8. Hepatitis B Training (if required); 9. Social Security Card and valid Driver's License (for identification upon hiring), to be copied by Personnel Office. BY SELECTING "YES", YOU AGREE TO PROVIDE /COMPLETE THESE REQUIRED ITEMS. I authorize Valley School District to make an investigation of my personal, educational, vocational, or employment history. I further authorize any current or former employer, firm, corporation, educational or vocational institution or government agency to provide Valley School District with any information they have regarding me. I hereby release and discharge Valley School District and all those who provide information from any and all liability as a result of requesting, furnishing and receiving this information. I understand that references and personal information, which become part of this application will be regarded as confidential and will not be revealed to me.
