UW Medical Center-Montlake - General Internal Medicine and OB/GYN and Primary Care Clinics at Roosevelt have an outstanding opportunity for a Registered Nurse Care Manager (job profile: Registered Nurse 2).
WORK SCHEDULE
Full time / 40 hours per week
8:00am - 5:00pm, Monday - Friday
The RN will split their time between the General Internal Medicine Clinic (GIMC) and the OB/GYN and Primary Care Clinic at Roosevelt
Our clinics are closed on weekends and holidays
DEPARTMENT DESCRIPTION
The General Internal Medicine Clinic (GIMC) is a dynamic teaching clinic that provides patient-centered care with the best available medical evidence. The GIMC provides comprehensive services for adult patients, including behavioral health care, chronic disease management, preventive medicine, and primary care services.
Our OB/GYN and Primary Care Clinic provides primary care and specialized care for a wide breath of gynecology & obstetrics needs. This position will specifically focus on the primary care patient population.
Both clinics are located at our Roosevelt Medical Center in the Seattle University District.
POSITION HIGHLIGHTS
Empower patients to navigate their treatment plans
Provide guidance and advocacy throughout the care continuum
PRIMARY JOB RESPONSIBILITIES
Supports patients in avoiding hospital admission/readmission by providing timely outreach to assess the patient's health care needs, providing education on their disease process and treatment plans, and setting realistic goals to achieve optimal health
Provides episodic and ongoing care management services for a panel of patients with complex care needs
Partners with the patient to formulate, coordinate, and revise an individualized plan of care as indicated by the patient's response to treatment and evaluates overall plan for effectiveness
Works collaboratively with providers and staff across the UW Medicine system to ensure seamless transitions throughout the continuum of care
Assumes responsibility for continued professional growth and sharing expertise with other team members
REQUIREMENTS
At least 2 years of RN work experience
Recent care management experience in an outpatient setting, or nursing experience in primary care, internal medicine, or a similar practice area
Active licensure to practice as a Registered Nurse in Washington State
