Job description

The Case Manager of Youth Engagement Team provides case management and prevention services to systems-involved youth under the age of 18 and their families. They create housing stability through goal-based, strength-based, and participant-directed support.


The Case Manager of Youth Engagement Team secures and distributes financial support to prevent homelessness and advocates for clients, developing community linkages and resource referrals. They provide outreach to schools, businesses, and other organizations interested in supporting youth.


REPORTS TO: Supervising Therapist, Youth Engagement Team

Case Management Tasks

  1. Enroll youth and families facing imminent homelessness into case management as needed.
  2. Identify and create goals to meet client needs to prevent homelessness and stabilize housing.
  3. Document and update goals and progress in client plans.
  4. Provide necessary referrals to services or agencies designed to help clients meet their goals.
  5. Maintain timely and clear documentation in case notes, assessments, and referrals of all contacts with youth, families, and external service providers.
  6. Ensure youth experiencing homelessness are aware of the Coordinated Entry System (CEA) application process and assist with youth referrals accordingly.


Community Outreach and Project Collaboration

  1. Identify clients through outreach efforts and community partnerships by connecting with schools, shelters, teen centers, Childrens Administration, and other community services.
  2. Provide necessary referrals and information for services designed to help clients meet their goals, including temporary housing, employment, and community resources.
  3. Participate in Youth Engagement Team (YET) collaborative work, support strong communication and relationship development between YET project partners (YMCA, Childrens Administration, LCSYS, All Home staff, and advisory committee team members).


JOB REQUIREMENTS


A. Education and Experience

  1. Bachelor's Degree in a human service field required.
  2. Minimum oof 2 years of relevant working experience, including supporting youth, young adults and families experiencing homelessness, and systems that involve youth such as foster care and juvenile justice.
  3. Bilingual in English/Spanish preferred.


B. Additional Requirements


  1. Must satisfactorily pass criminal history check.
  2. Must satisfactorily pass tuberculosis (TB) test.
  3. Must have reliable transportation and a valid Washington State driver's license (or be able to obtain one by date of employment).
  4. Employees and volunteers who operate their own or Friends of Youth vehicles on Friends of Youth business, must have a safe driving record as defined by Friends of Youth.
  5. Employees and volunteers who operate their own vehicle(s) on Friends of Youth business must carry auto liability insurance that complies with the requirements of Friends of Youth.
  6. Must successfully complete the following health certification requirements within 30 days of employment: CPR, First Aid and Bloodborne Pathogens.
  7. Proof of COVID-19 vaccination is required. There are no exemptions.
