At Cocoon House we partner with young people, families, and communities in Snohomish County to break the cycle of homelessness through outreach, housing, and prevention. The Young Adult Housing Program, Journeys, is looking for a full-time Case Manager to join the team. Journeys provides 20 leased units of permanent supportive housing to households who are unhoused and between the ages of 18-24 at intake. Program staff support tenants in building skills needed to maintain a lease and live independently, as well as supporting tenants working toward self-identified goals and helping tenants build a support network outside of Cocoon House. The Case Manager works with tenants to maintain housing and build skills to support stability throughout adulthood. The Case Manager supports tenants in setting and progressing toward goals in various domains including basic needs, life skills, education, employment, health, mental health, and substance use. Tenants are not required to utilize case management services, so the ideal case manager will bring creativity and thoughtfulness to the role to encourage tenants to participate in skill building and other services - either formal or informal. This role carries the bulk of residents in the Journeys program, having a range of 10-15 clients at a time.

Young Adult Housing Case Manager Duties & Responsibilities

Full-Time On-site

Wage: $20-22/hr +$1.50/hr pay differential for multi-lingual skills with additional paperwork

This position works Tuesday-Saturday 12:30pm to 9pm. The role requires flexibility, at least one weekend day and some earlier days as needed.

  • Create a welcoming community and build relationships with tenants while maintaining appropriate boundaries.
  • Assist tenants in maintaining housing, including support around following a lease and maintaining units
  • Provide client-driven case management by supporting tenants in setting and working toward goals and skill development with emphasis on independent living skills, meeting lease requirements, and building external support networks
  • Find and connect tenants to needed support services in Snohomish County to facilitate long-term stabilization, ensuring tenants are able to maintain housing stability independent of Cocoon House staff support.
  • Collaborate with Second Case Manager and Residential Support Specialist to ensure that tenants are able to progress on their goals during hours Case Manager is offsite.
  • Support tenants to productively address issues and disputes within the program.
  • Conduct assessments of outcomes and goals at entry, during, and exit for each tenant. Complete all required paperwork and data entry on-time, accurately, and in compliance with internal and external database standards.
  • Maintain professional boundaries with residents while providing a supportive community environment.
  • Complete daily log notes to ensure consistent and clear communication between staff.
  • Attend meetings as required at program, agency, and community levels.
  • Maintain tenant confidentiality in all activities.
  • Perform light cleaning duties in communal areas, including but limited to: vacuuming, sweeping/mopping, and trash removal.

Requirements:

General:

  • High School Diploma or GED required with a minimum of one year working directly with young adults experiencing homelessness, or any combination of experience and education that support the needs of the position. Preference will be given to candidates with lived experience.
  • Must be 21 years of age or older.
  • Clear a background check with DCYF.
  • Possess a valid Washington State driver's license, insurance and reliable transportation.
  • Open commitment to serving, supporting, accepting and collaborating with individuals of all races, ages, genders, backgrounds, political opinions and beliefs, sexual orientations, criminal histories, citizenship or marital statuses and religious beliefs. Special consideration given to applicants with a demonstrated history of supporting and advocating for the LGBTQ community, women, people of color, immigrants or other historically marginalized or persecuted groups.
  • Strong and effective communication skills allowing for open, trusting relationships with the youth served as well as other Cocoon House staff and service partners at all levels.
  • Must be proficient in crisis intervention/de-escalation in order to provide proper care to youth and young adults who are experiencing trauma or are in crisis. Ability to effectively assess situations and access emergency services when appropriate
  • Be committed to and experienced with youth services.
  • Possess basic data recording skills and record keeping
  • Possess knowledge of street culture, adolescent development, trauma-informed care, and strengths-based approaches.
  • Must have good organizational skills and the ability to communicate with clients, vendors, and the public.

Physical:

  • Ability to effectively communicate with all internal and external clients
  • Be able to hear voices and alarms.
  • Ability to read and comprehend a variety of materials
  • Ability to operate phone, computer, and record information for files and logs
  • Must be able to lift up to 50 pounds
  • Ability to travel for various distances through urban or rural environments in various weather conditions
  • Must have ability to run up and down indoor and outdoor stairwells in case of youth emergency.

Compensation details: 20-22 Hourly Wage
