Applicant Information This position is not for a specific open position with the district. Your application will be reviewed and, if hired, you will be placed into a general pool of on-call substitutes that will be assigned for regular employees who are absent. Conditions of Employment Are you eligible for lawful employment in the U.S.? Proof of citizenship or legal right to work and identity will be required after hire. I authorize North Franklin School District to inquire with former employers or references and obtain any and all information regarding my job related background. I further authorize any person contacted by the North Franklin School District to provide information to the North Franklin School District. I understand that information from such former employers or references will not be made available to me. I release and hold harmless North Franklin School District, my former employers and all references from any and all liability arising from the obtaining of any information by, or the disclosing of any information, to the North Franklin School District or any of its agents. I certify (or declare) under penalty of perjury under laws of the State of Washington that the foregoing and all information included in the application is true and correct. If the information provided or answer(s) to any question on the application or the Pre- Employment Background Questionnaire change prior to my being hired, I understand that I must immediately notify the North Franklin School District. I understand I must answer this application truthfully and completely. Any falsification or deliberate misrepresentation, including omission of a material fact, in completion of this application can be grounds for denial of employment or continued employment. I understand that this application becomes the property of North Franklin School District. The District reserves the right to accept or reject it.
