Description:

OBJECTIVE/PURPOSE:
Under the direction of the Program Coordinator, the Direct Support Professional (DSP) is responsible for performing a variety of tasks related to the instruction and monitoring of individuals' employment skills for developing maximum independence and inclusion into the workforce. The DSP will provide direct supports to those with intellectual disabilities in community settings for employment opportunities.
MAJOR RESPONSIBILITIES:

1.0 DIRECT SERVICE:
Provides transportation to and from employment by privately owned vehicle or by agency vehicle. Teaches client job functions and specific task steps as needed with client independence on the job as the target goal. Implements methods to teach increased skill building. Facilitates communication and collaboration with client's supervisor encouraging increased independence. Facilitates personal relationships within the workplace promoting natural supports with co-workers. Encourages individuals to pursue their goals to make choices with employment pursuits. Acts as a role model for the individual and the employment community. Provides personal care assistance as needed. Keeps individuals safe across all settings. Follows schedules created by the Program Administrator and the Coordinator within the program. Acts as an advocate for clients across all settings. Utilizes a person's primary mode of communication, i.e., such as ensuring the use of a translator or sign-language interpreter, or facilitated communication (FC) as applicable, understanding the individual's unique communication style and/or responding in accordance with developmental need. Follows all support plans per Developmental Services (DS) policy. Completes all trainings as assigned. Works as part of a dynamic team.

2.0 RECORD KEEPING:
Completes all Developmental Services documentation in a timely and accurate manner including, but not limited to, billing appointments, progress notes, monthly summaries, and sequence sheets. Is familiar with each client's goal and writes quality, informative notes regarding progress and daily observations. Informs Coordinator and case manager if goal needs to be updated or changed.

3.0 NETWORKING:
Provides needed information to other staff, professionals, and family, as needed. Serves as advocate and/or liaison on behalf of individual served, the Developmental Services Division and UCS. Participates as a member of the individuals, Developmental Services, and UCS teams. Encourages and advocates for positive relationships with other community entities. Participates in treatment plan meetings and assists with establishing quality treatment plan goals. Encourages and ensures ongoing communication between team members.
In addition to the above, performs other duties as assigned.



Qualifications:

EDUCATIONAL/EXPERIENCE/CREDENTIAL REQUIREMENTS:
High School Diploma or equivalent. Relevant experience in human services field preferred.
LICENSE REQUIRED:
Valid Driver's License
