Job Summary: The Community Health (CHW) worker will work with underserved populations to facilitate health promotion and education, disease prevention, system navigation, as well as helping clients, families, and communities develop their capacity to access quality healthcare and social capital assets. The CHW serves a bridge between underserved communities, local churches and other faith based agencies, Wesley Nurses and other MHM partners, and community health providers. Essential Duties: Maintains up to date knowledge and understanding of community and MHM partners, services, resources and assets that can help improve health outcomes for underserved populations. Identifies, engages and connects underserved populations to healthcare resources and services. Aids in the creation and maintenance of community partnerships, programs and services. Establishes and promotes healthy positive relationships between clients, families and healthcare providers. Provide informal counseling, support, and follow-up for clients/patients. Effectively works with clients, Wesley Nurses, community partners, healthcare agencies and other providers from diverse backgrounds in reducing cultural and social economic barriers to quality healthcare. Helps clients, families and communities build social capital and efficacy to support positive health outcomes. Translating and interpreting for clients and healthcare/social service providers. Maintains appropriate records and statistical data necessary to document their functions and activities. Performs other duties as assigned. Education and/or Experience: High school diploma or general education degree (GED) preferred. Knowledge of community and 1-2 years of experience as a community health worker preferred. Certificates, Licenses, Registrations: Valid certification as a Community Health Worker by the Texas Department of State Health Services preferred. CPR certification preferred. Language Skills: Must be able to read and interpret documents such as safety rules, instructions, and procedure manuals. Must be able to write routine reports, and correspondence. Must be able to speak and present information effectively before groups of diverse customers. Must be fluent in English and Spanish. Reasoning Ability: Must be able to define problems, collect data, establish facts, and draw valid conclusions. Must be able to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Computer Skills Must be able to demonstrate working knowledge of the Internet, Outlook, Word and Excel applications Other: Must be able to drive and have access to a vehicle; maintain valid driver's license and auto liability insurance Work Environment and Physical Demands: The work environment characteristics described here are representative of those a team member encounters while performing the essential functions of this job. While performing the duties of this job, the team member is exposed to an office environment having direct contact with clients and the public. Position necessitates bending, stooping, twisting, turning, walking, climbing step stools, sitting and standing for periods of time. Must be able to lift and maneuver at least 30 pounds.
