Client location and schedule hours:

Client is located on Burkitt Rd. The schedule is Monday -Sunday 8 am to 8 pm.

Job Responsibilities:
Diagnoses: Post 10-11 Mini Strokes, Diabetes, Pancreatic Cancer Allergies: None known to Food or Medicine
Goals and Objectives: RickClient needs assistants with a gait belt on all transfers out of bed and up onto his walker. Observe and walk with him to restroom or bed or chair. Prepare, serve & encourage meals/snacks. Bed baths every other day. Laundry and bed linens as needed. Remind and assist with his meds. keeping log of any pain med doses. Offer and assist with fluid intake. Assist with dressing and grooming when he wants to wash hair or shave. Accompany with wife to treatments or other Dr appts when needed. Light housekeeping. Engage with often.

ADL's


Mobility: Client needs assist with a gait belt to get out of bed and to get up onto his walker. Once on his walker he can take steps. He will be getting PT and OT when he gets home in an effort to improve his mobility. He will need full assist to get into a vehicle for appts.

Toileting: Once on his walker, he can walk into the bathroom. He does recognize when he needs to use the restroom. For any urgent needs, He will need help staying clean & dry, changing depends as needed and needs help wiping.

Eating: Client can feed himself but is not often hungry and is getting particular about what he eats since he says food has no real taste to him. He needs to be encouraged to eat meals and snacks and to stay hydrated. Milkshakes (low carb or low sugar) may be good options. Diabetic Ensure also may work.

Bathing/Dressing: Client needs help changing his clothes each day but he can decide what he wants to wear. He will need bed baths every other day or as he requests them. He will need linens changed also as needed or weekly. Laundry as needed. He is a full assist when able to get showers.

Communication: Client can speak clearly and is easily understood. His receptive skills are good, with no hearing loss, but his response time appears slightly delayed sometimes but it will be appropriate.

Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
TB test
