Are you looking for an excellent opportunity to work with a faith-based non-profit organization? Lutheran Hospice is currently recruiting for a Licensed Master Social Worker.

We are seeking someone who is a compassionate, dedicated, and caring professional to join our team!

Responsibilities:

On an ongoing basis, assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment.

Completes an individualized initial plan of care based on immediate needs and completes this documentation within the required timeframe.

Completes admission paperwork in an efficient and timely manner ensuring that the caregiver/patient understands the information provided.

Helps to ensure seamless service delivery as a member of the social work staff. In addition to admission/referral responsibilities, assists with emergency visits and routine visits as necessary to meet the needs of patients and caregivers.

Provides anticipatory grief counseling services to patients and their caregivers according to the established plan of care and as ordered by the physician.

Helps patients/caregivers in understanding the philosophy of hospice and the dying process.

Carries out psychosocial evaluations and plans interventions based on evaluation findings. Counsel's patient and family/caregivers as needed in relationship to stress and other identified coping difficulties.

Provides leadership in strategic planning including identifying opportunities for additional or improved services to address unmet customer needs.

Provides advocacy and assists patients/caregivers with identifying community resources for nursing home placement, funeral home planning, etc.

Assists in managing discharge planning and processing when appropriate. Assists patient and family/caregiver with planning for end of life needs.

Qualifications:

Current MSW licensure for the state.

A graduate of a Master's program in social work accredited by the Council on Social Work Education preferred.

Minimum of one year of experience in health care, with hospice experience preferred.

Understands hospice philosophy, and issues of death/dying.

Demonstrates good verbal and written communication, and organization skills.

Demonstrates strong public relations skills.

Demonstrates autonomy, organization, assertiveness, and flexibility and cooperation in performing job responsibilities.

Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Must be able to travel throughout the service area.
