As a Certified Nurse Aide for Homeland Hospice, you would contribute to our community of caring by providing direct support to patients and family members during a significant part of their life journey.

- This is a full-time, Monday through Friday, daytime position.
- This position requires a valid Pennsylvania CNA license, a valid Pennsylvania driver's license, and reliable internet access.

As Homeland Hospice's CNA you would provide personal care and light housekeeping services to terminally ill patients in their home, nursing facility, inpatient units or other environments where the Hospice patients reside, in accordance with an established plan of care and under the supervision of a Registered Nurse (RN). You would actively participate as a member of the Hospice Interdisciplinary Team (IDT) and provide care and comfort to patients with assigned tasks following the established IDT and Hospice Aide plan of care. You would observe the patient, report and document observations and care performed and provide timely documentation of services based on the patient's care plan.
CNA Requirements include:
- Education and Experience Requirements:
- Minimum age requirement of 18.
- Minimum of one (1) year of direct patient care experience in an institutional or home care setting required with community health/hospice experience preferred.
- Ability to work within an interdisciplinary setting.
- Understanding of Hospice philosophy and needs of terminally ill.
- Completion of an approved hospice aide training program as outlined under the Hospice Conditions of Participation 418.76: Hospice Aide/Homemaker Services; and proof of certification as a Hospice Aide from the program.

Completion of a nurse aide training and competency evaluation program approved by the State as meeting the requirements under Medicare 483.151 through 483.154 and is currently listed in good standing on the State nurse aide registry.

Completion of a state approved training program and certification as a home health aide, and proof of certification. - Must demonstrate skills and have a passing score on a written competency test in accordance with 42 C.F.R. Section 484.36 prior to working independently with a patient.
- Licensure, Registration, and Certification Requirements:
- Valid Pennsylvania CNA Certification.
- Valid driver's license and reliable transportation that is insured in accordance with Pennsylvania state requirements and agency policy.
- Satisfactory references from employers and/or professional peers.
- Satisfactory criminal background check.
- Maintains current CPR certification.
- Access to reliable internet access.
