Registered Nurse - Supervisor/ Wound Care Nurse
must be willing to work occasional nights and rotate weekends.
Required Skills
Graduate from an accredited program of professional nursing
Currently licensed as a Registered Nurse in the Commonwealth of Pennsylvania and in good standing
Criminal Background check current within 1 (one) year
Preferred Skills
1 years experience in the geriatric field preferred
CPR Certification preferred
IV Therapy Certification preferred
Wound Care certification preferred
Job Summary:
We are seeking a highly organized and detail oriented Registered Nurse to join our team. The Registered Nurse/Wound Care Nurse effectively interacts with residents, their significant others and the health care team members in accordance with St. Anne Home's ethical requirements as described in our Mission, Vision, Value, Pledge and Fundamental Commitment statements found in our Code of Corporate Compliance follows on a weekly basis all residents who present with alterations in skin integrity including but not limited to; pressure, venous, arterial, and diabetic ulcers as well as surgical wounds. The Wound care Nurse will assess all new admissions to identify risk factors for skin breakdown and develop a plan of care according to those risk factors. There will be a collaboration with the Doctor and other wound care professionals to ensure an optimal plan of care is in place. The Wound Care Nurse will complete MDS sections in relation to skin and wound care
Responsibilities (include but are not limited to):
Nursing focus
Uses the nursing process (assessment, planning, implementation and evaluation) to provide resident are
Performs physical, psychological and social assessment of residents
Demonstrates skills and knowledge to provide safe, competent nursing care
Participates in resident teaching activities
Conduct inventories, administer medications and treatment, as well as record Schedule I medications
Resident Care focus
Supervisors/delegates resident care on assigned unit.
Obtains physician orders and carries out the physician orders.
Charges equipment, supplies and services correctly.
Participates in quality assessment and performance improvement activities (QAPI) as appropriate.
Participates in nurse aide evaluations as necessary.
Responds appropriately to emergency situations
Wound Care focus
Provides consultation for individuals with altered skin integrity (eg, pressure ulcers, draining wounds, fistulae) to develop individualized plans of care and attain expected outcomes.
Provides guidance to staff in implementing protocols to identify, control, or eliminate etiologic factors for skin breakdown, including selection of appropriate support surfaces.
With a physician's order, provides appropriate debridement of devitalized tissue (eg, conservative sharp debridement, silver nitrate cauterization of non-proliferative wound edges, hypergranulation tissue, or to control minor bleeding).
Provides appropriate education to patients, caregivers, and staff regarding skin care, wound management, care of percutaneous tubes, and draining wound/fistulae management.
Validates pressure ulcer data collection for nursing quality indicators (eg, pressure ulcers and restraint use)
Provides pre- and postoperative education to patients (and their families) who are undergoing ostomy surgery, with consideration of the need for physical and psychological adaptation.
Marks the stoma site preoperatively by determining the appropriate site for stoma placement with consideration of the patient's anatomical markings, physical capabilities, and lifestyle.
Assesses the stoma and stoma functioning and initiates appropriate procedures for stoma care (eg, removes rod/sutures, measures/fits a pouching system, teaches care of stoma/peristomal skin and pouch).
Provides appropriate educational information to patients, including postoperative and discharge instructions about care, referrals, and supply needs
Identifies risk factors for urinary or fecal continence disorders.
Assesses patients with urinary and/or fecal continence disorders.
Establishes an appropriate management program to include dietary and fluid management; bowel training or stimulated defecation program; bladder retraining, prompted voiding, or a scheduled voiding program; pelvic muscle reeducation without biofeedback; indwelling catheter management; recommendations regarding containment/absorptive products and devices and skin care; and education and counseling for patients/caregivers.
Identifies patients requiring referral for assessment/management of complex urinary or fecal continence disorders.
