Rates starting at $24.50 per hour.

We are offering a $6,000 Sign-On Bonus (to be paid throughout the first year of employment).

Licensed Practical Nurse is a direct patient care position providing nursing care under the supervision of the Registered Nurse.

Provides instruction to patients/family as established by the RN and outlined in the plan of care to promote independent functioning and health promotion.
Supports patient through physical and emotional changes.
Advocates for the wellbeing and wishes of the patient to the RN.
Follows the organization's policy regarding the confidential treatment of patient, employee, and proprietary information (including HIPPA).
Complies with safety policies and procedures at all times for patients, families and coworkers, and informs the Manager of any unsafe conditions.
Includes the patient's and family's safety as a concern when executing the plan of care.
Follows basic infection control procedures and uses standard precautions.
Updates RN and other team members regarding patient's condition/status so as to facilitate excellent, comprehensive care and positive patient outcomes.
Regularly attends and participates in team and patient care conferences.
Handles all problems and concerns with the highest level of professionalism, tact and respect.
Discuss appropriate customer responses with the Manager to ensure a proactive approach to working with our clients concerns.
Acts as a liaison to resolve all customer concerns and/or problems between the organization and clients.
Performs other duties as assigned.

Currently licensed as a Licensed Practical Nurse in the Commonwealth of PA.
Must have dependable transportation with a valid driver's license. Independently visit patients in their homes.
