SUPERVISION RECEIVED This position reports directly to the Aging Care Manager or Aging Care Supervisor. SUPERVISION EXERCISED This position does not exercise supervision over other functions. ESSENTIAL DUTIES OF THE POSITION Visits applicants for Area Agency on Aging services in their homes to complete assessments and determine eligibility. Provides detailed explanation of services for which applicants appear eligible. Discusses any problems in obtaining services, and make appropriate referrals, escalating complex situations to the Supervisor. Reviews completed vouchers sent to Area Agency on Aging for allowability of services/items purchased by Caregiver for Caregiver Support Program. Performs follow-up to ensure service delivery begins and reassessments of consumers as required. Maintains timely documentation in the electronic system of record, as well as hard copy files. Attends supervisory conferences/Unit meetings as scheduled. Attends training, locally or out-of-town, as approved by the Supervisor. Cooperates with co-workers and staff of other Division of the Department of Human Services and community agencies in the provision of service to shared consumers when appropriate. Performs related duties as required. An employee assigned to this title shall perform a majority, but may not perform all, of the duties listed in this job description. REQUIRED MINIMUM QUALIFICATIONS EDUCATION & EXPERIENCE - One (1) year as a Case Aide 1 (Local government); OR Two (2) years of public contact work, including one (1) year in a human services program; OR Any equivalent combination of experience and training. Employee assigned to this title will be required to possess and maintain a valid and current motor vehicle operator's license. Must have an acceptable driving record. Personal vehicle required for County business, employee is eligible for mileage reimbursement. KNOWLEDGE, SKILLS, AND ABILITIES Knowledge of the physical, cultural, and social aspects of the assigned community. Ability to obtain, evaluate, and analyze information from interviews, reports, records, and other type of sources of information, and to provide information to clients and other types of personnel. Ability to establish and maintain positive working relationships. Ability to learn about Area Agency on Aging Programs and the services of other community agencies. Ability to communicate verbally with older persons. Ability to perform simple arithmetic computations. Ability to follow simple oral and written instructions. Ability to express ideas clearly and persuasively to elderly clients. Ability to clearly express ideas verbally, and in writing. Ability to maintain confidentiality at all times. Ability to communicate fluently in English, including telephone contacts, face-to-face contacts, and communications in large and small group settings. Ability to read the English writing in cursive and printed forms. Ability to analyze and interpret information that is received in written and verbal form. TOOLS AND EQUIPMENT Telephone, personal computer (including word processing and spreadsheet software), calculator, writing implements, fax machine, copy machine, and paper shredder. PHYSICAL DEMANDS While performing the duties of this job, the employee is frequently required to walk, sit, talk or hear. The employee must occasionally lift and/or move up to twenty-five (25) pounds.
