Job description
Position Overview:
St. Charles Health System is seeking a highly skilled Structural Heart Advanced Practice Provider (APP) to collaborate with other medical specialties and provide exceptional care for patients with structural heart conditions. This role includes comprehensive patient care across multiple phases screening, pre-, intra-, and post-procedural care along with hospital rounding, discharge planning, and clinic visits. The position is based primarily at St. Charles Bend Hospital, with outpatient work at the Bend Heart & Lung Center.
Work Schedule:
Full-time position with a requirement of 156 clinical shifts per year.
Duties include patient assessment, care management, and documentation.
Blend of hospital and clinic work, including pre- and post-procedural care, hospital rounding, and outpatient visits.
Call Expectation:
Participation in the Structural Heart APP call rotation.
The APP provides first-line overnight support for admitted patients and responds to outpatient after-hours calls.
Will assist in urgent or emergent surgical procedures when activated within the facility's onsite response time.
Essential Duties and Responsibilities:
As a Structural Heart APP at St. Charles Health System, your responsibilities will include:
Clinical Responsibilities:
Comprehensive Patient Care: Conduct exams, diagnose, and manage a wide range of structural heart conditions.
Procedure Assistance: Assist in pre-, intra-, and post-procedural care for procedures like TAVR, TEER (Mitral and Tricuspid), LAAO, TTVR, and TMVR, as well as other cardiac interventions.
Diagnostic and Risk Assessment: Order and interpret diagnostic tests (labs, imaging) and utilize tools like the STS risk calculator to assess patient suitability for surgical interventions.
Medication and Treatment: Prescribe and administer medications according to state law and clinical guidelines.
Patient Education: Provide education on health, illness prevention, and treatments to patients and their families.
Documentation: Maintain clear and concise patient records and ensure accurate documentation in the Electronic Medical Record (EMR).
Care Coordination: Work with multidisciplinary teams to provide patient-centered care and manage complex cardiac patients.
Administrative Responsibilities:
Complete all necessary documentation, including billing, in accordance with health system policies and regulatory guidelines.
Participate in St. Charles Health System and SCMG physician meetings, contributing to policy formation and continuous improvement efforts.
Stay current on medical knowledge and skills through Continuing Medical Education (CME).

Education & Certifications:
Successful completion of an accredited Nurse Practitioner or Physician Assistant program.
Unrestricted licensure as a Nurse Practitioner or Physician Assistant in the state of Oregon.
NCCPA certification for Physician Assistants; AANC, NCC, or AANP certification for Nurse Practitioners.
Current ACLS certification.
Ability to maintain an unrestricted Federal DEA number in Oregon.
Experience:
Required: Experience working in cardiac hospital and outpatient environments, coordinating complex patient care.
Preferred: 2-5 years of experience in structural heart care, including cardiac procedures and patient management.
