Requirements:

• OH License
• 2 Years' Experience
• Associate's Degree in Nursing.
• BLS
• 3 years home hospice experience preferred
• Community Hospital Experience preferred
• LTAC Experience preferred

• Traveler's permanent address must be greater than 100 miles from the most central facility
• Travelers are first to float

JOB SUMMARYIs responsible for making home visits to assigned patients requiring skilled intermittent professional nursing services in the home. All findings and/or changes regarding patient are communicated to the following physician.
MINIMUM QUALIFICATIONS (3281):Minimum Qualifications include: Valid state driver's license, Minimum of eighteen (18) years old, Have an acceptable Motor Vehicle Report (MVR), Must provide proof of insurance, if applicable - Associate's Degreein Nursing. Current RN licensure in the state of Ohio
3 years home hospice experience preferred
