Immediate Eligibility for Holiday Pay

Education & Experience:

License Practical Nurse (LPN) in the state of Ohio
Associate or Bachelor of Science Degree in Nursing or in related field
Must be CPR/First Aid Certified
Ohio driver's license, current auto insurance and access to a car during working hours.
Home Health experience with terminally ill patients preferred.

Strong ability to multitask and work in a fast-paced environment
Excellent skills in leadership, management, interpersonal relations, networking, family, and customer relations.
Strong verbal and written communication skills.
Demonstrate flexibility, excellent assessment skills, and ability to supervise others and the confidence to work independently.
Demonstrate skill in nursing practice, ability to cope with patient/family emotional stress and tolerance of individual lifestyles.
Active listening skills with the goal of understanding
Possess skills of working with team having positive outcomes

Job Duties:

Assume responsibility for patients and families under the care of Zusman Community Hospice. May include assessment, planning, implementation and evaluation phases of the nursing process.
Initiate communication with attending physicians, other hospice team members and agencies as needed to coordinate optimal care and use of resources for the patient/family.
Maintain up-to-date records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
Perform other related functions as required by the Director.
