Report to the Coordinator, day-to-day regarding the operation of the Day Habilitation Program.

Responsibilities:
Specific duties include the following but are not limited to:

  • Provides leadership, direction and supervision to the Assistant Day Habilitation.
  • Supervisor and Habilitation Counselor in the operations of the program.
  • Provides leadership and direction to the Individuals and staff in planning and development of appropriate and accurate Habilitation plans that reflect personal interests and needs.
  • To ensure the health, safety, and well-being of the Individuals we serve.
  • Serves as a liaison between agency, community, volunteer sites, advocates and families.
  • Serves as an advocate and provider for all issues pertaining to individuals receiving services. Meets with representative at volunteer sites to discuss progress and concerns.
  • Ensuring Medicaid compliance and knowledge of updated regulation as it relates to Day Habilitation, and Staying abreast of OPWDD regulations.
  • Implements active treatment plans for Individuals, both inside and outside of building.
  • Participate in the hiring process of new staff. Provide direct supervision to all Day Habilitation Counselor. Train evaluate and disciplining of staff. Provide direct supervision to all Day Habilitation Counselors.
  • Facilitate and attend regularly scheduled supervisory staff meeting. Facilitate in scheduled in-service training programs. Facilitate special IDT'S, Annual and Semi-Annual. Complete all meeting minutes for scheduled meetings.
  • Developing activities for program/ implementation of plans and community integration.
  • Complete accurately the following routine written work: Day Habilitation Plans, documentation of services delivered contemporaneously, attendance, progress notes, any documentation required by supervisor and/or administration.
  • Responsible for maintaining the physical up keep and compliance of the program. Complete physical plant reviews and follow up on repairs.
  • Ensure that documentation are filed and kept audit ready at all times.
  • Screen, accept, follow up on all new individuals to Day Habilitation, and maintain census of all enrollment of Day Habilitation.
  • Provides outreach to the community to create opportunities for the Individuals we serve to be involved in the community and to integrate within the community.
  • Models a professional demeanor with the Individuals while in the community.
  • Serves as an Agency representative while out in the community.
  • Maintains appropriate contact with the facility during the day regarding location, events of the day any concerns.
  • Maintain on going communication with staff throughout the day.
  • On-going communication with families and residential sites for all enrolled individuals.
  • Visits volunteer sites and evaluate for appropriate placement for our Individuals.
  • Notifies administration and nurse of any medical concerns of the persons we serve.
  • Ensure that referrals are made to appropriate internal and external services as required to meet the Individual's needs.
  • Participate in cross program service reviews that support the planning and coordination of service delivery and service closure.
  • Facilitate staff meetings and trainings to ensure compliance with Agency and OPWDD regulations.
  • Perform other related duties as requested.
  • Ongoing staff training to ensure proper documentation of billable activities.
  • Ensure data collected matches day habilitation goals.

Qualifications:
A minimum of Bachelor's Degree
2 years of experience with MR/DD preferred

Experience :
  • Knowledge of Home and Community Based Waiver, and the ability to keep up to date pertaining to such knowledge.
  • Understanding of Persons Center Planning.
  • Reasonable flexibility in work hours and ability to respond to the scheduling needs of the individual and or program training.
  • Successful completion and proficiency of SCIP Training, and CPR/First Aid.
  • Ability to follow directions well and exhibit good decision-making, and communication skills, both verbal and written.
  • Ability to organize work with self and others.
  • Ability to establish and maintain a working relationship with Individuals, families, staff and community agencies.
  • Must be able to take initiative and have the ability to problem solve, handle emergency/crisis situations.
  • Must be reliable, dependable and flexible and have the ability to evaluate the individuals' needs.
  • Must make on site observation at the volunteer location
  • Valid Driver's License required.
