Job Description:

Summary of Duties and Responsibilities:

Patients transitioning out of the acute inpatient care setting including those discharged to home, shelter, or skilled nursing settings, among others remain among the most vulnerable to experiencing fragmentation and gaps in care, increasing risk for preventable adverse outcomes and downstream costs of care. Safety net hospitals and health systems face unique challenges in delivering effective transitional care in the post-inpatient period, serving a patient population with an overall higher risk of readmission and greater unmet behavioral health, economic, housing and social needs. Meeting these needs requires strong collaboration, coordination, integration, and management of care planning and delivery across disciplines within and beyond the hospital inpatient setting and outside the health care sector.


Across NYC Health + Hospitals, the Transitional Care Management (TCM) program is a standardized and systemwide program that aims to support successful transitions for high-risk patients from hospital inpatient to community settings. Within the program, Transitional Care Coordinators (TCCs) and Transitional Care Managers work collaboratively alongside interdisciplinary teams to perform transitional care activities, including in both discharge planning and post-discharge monitoring phases. During the inpatient admission, the patient is screened for root causes of admission, unmet medical or social needs, and other re-admission risks. Staff then work with patients, outpatient caregivers and providers, and the inpatient interdisciplinary team to develop and implement a care plan customized to the patient s needs. Upon discharge, transitional care staff monitor the patient with regular callbacks for 30 days post-discharge. Though operationalized by staff embedded within care teams at each acute care hospital, the TCM program is a systemwide program leveraging standardized operational guidance, workforce training, program policies, and monitoring and evaluation tools.


The Training Director for the Transitional Care Management (TCM) program will be responsible for developing and implementing a comprehensive training and workforce development program across our 11-hospital system, aiming to ensure that TCM staff, including Transitional Care Coordinators (TCCs) and transitional nurse care managers, are equipped with the skills and knowledge to deliver high-quality transitional care services. Reporting directly to the Program Director of TCM, the Training Director will collaborate closely with the TCM Program Director, the Central Office TCM implementation team, and other Central Office training stakeholders to design training curricula, produce operational guides and educational materials, deliver in-person and virtual training programming, and oversee continuous professional development. The Training Director will play a key role in enhancing staff competency, standardizing practices, fostering a culture that is patient-centered, culturally sensitive and trauma-informed, and improving patient outcomes through multifaceted and innovative training strategies.

The responsibilities of the Training Director will include but are not limited to:

  • Design and direct a comprehensive systemwide training and workforce development program for TCM staff, ensuring alignment with organizational goals, regulatory requirements, and best practices
  • Collaborate with internal and external stakeholders to develop orientation programs, training curricula and operational guides, utilizing various pedagogical approaches, including didactic sessions, workshops, case studies, experiential learning and online programming
  • Develop ongoing professional development programming that facilitates continued development of TCM staff skills and knowledge, including coaching initiatives
  • Develop and directs studies to determine workforce planning needs for TCM and potential shortage areas, and advises on workforce requirements and availability
  • Collaborate closely with Central Office TCM Implementation Team and facility TCM program directors to support roll-out of standardized TCM program, leading efforts from a training perspective
  • Work closely with facility TCM program directors to ensure consistent implementation of training programs and adherence to TCM model standards
  • Partner with Central Office departments (e.g., Office of Population Health, Office of Patient Centered Care) and other external agencies and stakeholders to develop and integrate TCM training programs with training initiatives
  • Develops, reviews and evaluates training programs and recommends priorities in allocation of available training resources, including by monitoring and assessing training effectiveness through regular evaluations and feedback mechanisms
  • Develops and directs Wellness initiatives, including through Helping Healers Heal, counselling and other assistance for participants in training and career development programs
  • Foster collaboration with the Epic team to optimize EHR functionalities supporting TCM workflows and documentation
  • Utilize data analytics to monitor key performance indicators related to training outcomes and staff competency
  • Regularly visit with all system acute care hospitals to identify and support the needs of facility, TCM leadership and their staff, and identify training gaps, providing hands-on training to facility staff when needed
  • Participate in regular office hours for frontline staff, provide feedback and be able to address nursing and training related barriers to effective transitional care
  • Collaborate with the Program Director to hold quarterly meetings with each site leadership to assess gaps in training program, assess fidelity to the TCM, gain feedback, and summarize data on program implementation and impact
  • Lead quality improvement efforts to enhance training effectiveness and continuously improve transitional care practices
  • Stay informed about emerging trends and best practices in transitional care management through ongoing education, specialized training, and research activities
  • Directs informational activities relating to training, job opportunity and career programs within NYC H+H in order to assist personnel recruitment activities
  • Performs other duties as required


Minimum Qualifications:

  1. A Baccalaureate Degree from an accredited college or university in Education, Public Health, Psychology, Personnel Administration, Management or Public Administration or a related approved discipline; and,
  2. Eight (8) years of progressively responsible experience in planning and manpower development and training programs, medical or public health administration or other fields related to training, development and manpower analysis, of which at least three (3) years have been in a managerial, supervisory or administrative capacity related to the organization, formulation, development and implementation of such plans and programs; or,
  3. A Masters Degree from an accredited college or university in Education, Management, Social Work, Medical Administration, Psychology, Personnel Administration, Public Administration or a related approved field, with seven (7) years of experience as described in item and two (2) years in a managerial, supervisory or administrative capacity; and,
  4. Extensive knowledge of the principles of training and manpower development programs and ability to analyze and determine the needs of the Corporation; or,
  5. A satisfactory combination of training, education and experience indicating the ability to perform satisfactorily.
