We are looking for a Physical Therapists to cover Westchester, Putnam & Dutchess Counties.

Here are some general job responsibilities for our Physical Therapist (PT) position:

Assesses patients for functional limitations and develops a therapy plan of treatment with specific goals in collaboration with the patient, family, physician and other health team members.

Provides direct care to treat the medical/surgical condition through the use of accepted physical therapy modalities.

Instructs and supervises the patient/family and home health aide (and others) in the home exercise program and other aspects of patient care as appropriate and documents their response to instruction.

Provides written home exercise program to patient (and others) as needed and also provides a copy for the patient's record.

Continually re-evaluates the patient's condition and modifies the plan of treatment as appropriate.

Identifies and reports significant clinical information to the physician, Clinical Manager, primary nurse coordinating services and other disciplines.

Documents all clinical evaluations, treatments and patient's response to therapy interventions on a timely basis.

REQUIREMENTS:

NYS Professional License and current registration to practice as a Physical Therapist.

One (1) year of full time experience in direct provision of physical therapy service.

Home care experience preferred. Please note that salary and sign on bonus varies by hours worked (Full Time Employees are eligible for bonuses) plus years as a licensed PT as well as CHHA experience .

Automobile in good working order and auto insurance in the amount specified by VNSW. Valid driver's license.

Computer Knowledge preferred. Knowledge of HCHB Clinical Software preferred. Must maintain CEU requirements. Professional Liability Insurance in the amount specified by VNSW

Job Types: Full-time, Part-time

Pay: $95,000.00 - $108,000.00 per year
