Gurwin Home Care Agency - Gurwin Certified and Licensed Home Care Services Agencies are looking for a full-time Director of Patient Services (DPS).

Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin's Assisted Living community (Gurwin Jewish Fay J. Lindner Residences), our independent living community (Fountaingate Gardens).

EDUCATION AND EXPERIENCE REQUIREMENTS:

A current New York State Nursing license. A BSN from an accredited NLN program and a minimum of five years of progressively responsible experience in the provision of nursing services in a home health setting. Master's Degree in Nursing preferred. Advanced management, administrative and decision making skills congruent with responsibilities.

KNOWLEDGE, SKILLS AND ABILITIES:

Extensive knowledge of the principles and practice of community health nursing and its relationship to other human services. Superior clinical knowledge based on application, with in-depth understanding of case management and community resources/needs.

PRINCIPAL FUNCTIONS:

  • The Director of Patient Services is responsible for the administration of the program and the overall clinical direction and supervision of patient care services.
  • Assumes professional responsibility for the CHHA and integrated programs.
  • Provides leadership and direction of clinical staff for program services and policies of the organization. This involves planning, organizing, directing, monitoring and evaluating as well as assuring compliance with State, Federal and Agency standards.
  • Works closely with Executive Team, appropriate management staff and committees to facilitate program coordination and implementation.
DUTIES AND RESPONSIBILITIES:
  • Assists in recruiting and provides input to meet the program ' s responsibilities.
  • Plans, delegates, supervises and evaluates patient care services provided by the program directly, by fee-for service or by contract.
  • Provides clinical supervision of staff to assure standards of care are met and or assures that appropriate supervision is provided.
  • Carries out the CHHA mission, philosophy, goals and objectives within the guidelines of agency policy and position function.
  • Develop, review, maintain and assure implementation of clinical service delivery policies, procedures and objectives.
  • Collaborates with Human Resources and Director to monitor and assure that regulations and mandated requirements are followed.
  • Collaborates with and assists Director in the development, implementation and monitoring of the annual budget by recommending staffing plans, productivity volume estimates, as well as needed supplies and equipment that will accomplish program goals.
  • Assures the presence and implementation of staff development activities that include but are not limited to orientation, performance evaluation, PAC/QAPI, and continuing in-service education.
  • Serves as a clinical resource to the Executive Team.
