Our team is comprised of more than 1,500 primary care and behavioral health clinicians in more than 31 states. Our post-acute care clinicians receive best-in-class orientation and training, a dedicated EHR system and access to ongoing clinical education.

For this opportunity we ask that you be board certified in family medicine (FM) or internal medicine (IM), have a passion to serve active older adults, and a desire to lead.

Opportunity Overview:

  • Coverage area for full-time will be South Bend, Niles, and St. Joseph (3.5-5 days)
  • Coverage area for part-time will be South Bend, IN and St. Joseph, MI (1-3 days)
  • 15-20 patients per day
  • Mileage reimbursement
  • Medical directorship stipend
  • Uncapped productivity bonus

Responsibilities:

  • Drive high-quality, safe, and cost-effective patient care in post-acute care communities
  • Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
  • Work closely with hospice, palliative care, and home care agencies for safe patient transitions
  • Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines.
Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated salary range of $114,000 to $300,000 annually with no cap on productivity income potential and medical directorship.
