Primary Care Physician
Schedule: Exempt, min 40 hours/week participates in a 24-hour on call rotation with other Primary Care Providers.
Location Department: Huron Valley PACE, Ypsilanti
Overview:
The Primary Care Physician is responsible for providing primary medical care to participants with the highest medical standards and ethics in caring.
What you'll do:
Clinical Assessment and Care Planning
Perform initial comprehensive history and physical assessments on new enrollees, along with interval assessments for established participants (semi-annually or as needed).
Document assessments, care plans, and updates in the medical record in compliance with LifeCircles policy.
Develop participant care goals based on medical and psychosocial conditions, reflecting the individual's wishes and preferences.
Interdisciplinary Team (IDT) Collaboration
Actively participate as an IDT member, communicating participant updates and attending regular IDT meetings.
Assist in the development, implementation, and evaluation of individualized care plans in collaboration with other team members.
Coordinate care across the team and ensure seamless delivery of 24-hour services.
Medical Treatment and Health Maintenance
Evaluate and treat episodic illnesses, adjusting medications in response to health changes and in accordance with policies.
Provide participant and family education on chronic condition management and support family conferences.
Judiciously utilize specialty consultations and coordinate with medical specialists when needed.
Hospital and SNF Coordination
Communicate with attending physicians during participant hospitalizations, advocating for care that aligns with the participant's values and wishes.
Coordinate discharge planning with the IDT and hospital planners, facilitating smooth transitions.
Manage participant care in skilled nursing facilities (SNF), conducting regular evaluations and maintaining communication with SNF staff.
Documentation and Compliance
Maintain accurate, up-to-date documentation of diagnoses, health services provided, and care coordination.
Address inquiries from medical records staff and coding specialists promptly.
Participate in Quality Improvement activities and serve on relevant committees and work groups as needed.
On-Call and Travel Requirements
Participate in a 24-hour on-call rotation with other Primary Care Providers to ensure continuous participant support.
Conduct home visits as necessary and travel to various Huron Valley PACE locations, including Ypsilanti and Monroe.
Collaboration with Medical Leadership
Work closely with the Medical Director and Advance Practice Professionals in providing medical care, ensuring that participant's needs are met in a holistic, coordinated manner.
What it takes:
Graduate of an accredited School of Medicine or Osteopathy.
Board certified in Internal Medicine or Family Practice.
Unrestricted, valid license to practice medicine in the State of Michigan.
A valid driver's license is required.
Advance certification in geriatrics and/or palliative medicine is preferred.
Minimum of one-year prior work experience working with frail and elderly.
