Licensed Practical Nurse Manager

72,500.00 - 75,000.00

We are looking for a compassionate LPN to join our growing team! We are a family-owned company. The right candidate has a can-do attitude, excellent interpersonal skills, and a current state LPN license. In this role, you will be providing medical support to our clients while ensuring a safe environment for them.

Responsibilities

Assess residents in home, hospital, rehab, and other settings to determine initial care plans and appropriateness for move-in
Complete re-assessments at 30 and 90 days, as well as when there are significant changes and conduct care conferences with residents and families to discuss care changes.
Manage all Resident Care Staff, including completing education, discipline, and performance reviews.
Communicate with ancillary staff and coordinate continuation of care services.
Oversee the Infection Prevention and Control program and lead quarterly meetings.
Oversee Incident Management and Quality Review Program
Manage all aspects of resident care to ensure that residents are well cared for and treated with dignity and respect.

Qualifications

1 year experience as an LPN in a home care setting
Current state license as an LPN/RN
Proof of current CPR certification
Ability to work flexible schedule.
Excellent interpersonal skills
Valid driver's license and reliable transportation
