Job Description:

Oversee patient care for identified case load including providing primary and specialty group and individual services, coordination of treatment, referral and collateral contacts, treatment planning, case management and biopsychosocial assessment to ensure the patient enjoys a cohesive treatment experience where all are working together toward the same goal.
Provide individual and group counseling, crisis intervention, as well as education and skills training.
Ensures orientation of new clients immediately upon admission.
Obtains history and presenting information as delineated in program policy.
Collaboratively develops and updates an individualized treatment plan with the individual that includes targeted discharge date and plan.

Ability to work onsite
Knowledge of EMR system, Microsoft Office Programs (i.e. Excel, Word, Outlook, etc)

License/Certification: LMHC, LICSW, LCSW in MA.

Education/Work Experience :
Master's degree in the mental health field from an accredited institution.
Experience in mental health and addiction environments
