Job description
DUTIES OF POSITION:
Maintain the quality of patient care in accordance with the Agency's objectives and policies, through
planning, coordination, and implementation and evaluation of the home health services provided.
POSITION RESPONSIBILITIES:
Coordinates and schedules the daily work load of each visiting nurse to their fullest productivity
Available at all times during operating hours.
Assists with Administrator On-Call Rotation every 4-6 weeks
Provides daily supervision of nurses and home health aides to ensure the quality of care and home
care standards are met.
Reviews new admission records to assess disciplines needed, content, orders are present and the
appropriate care is provided.
Directs nursing and home health aide staff towards the appropriate use of community resources.
Appraise staff performance during their probationary period and annually with the Nursing Director,
provides the teaching and counseling necessary to enhance delivery of services and the professional
development of the nursing and home health aide staff.
Assist with orientation of new professional staff and familiarize them with policies and procedures.
Visit patients in the home care setting as needed.
Participates in audits, QAPI, interdisciplinary team conferences, morning meetings and nursing
meetings.
Reviews each nurse's schedules for recertification, discharges, LPN and Aide supervisions.
Reviews each nurse's end of day to better understand changes with patients and their care.
Meets with each nurse for case conference as needed.
Review weekly schedule submitted by nurses for changes or updates.
Completes the patients re-hospitalization referral/admission form (RCA) daily and submits it to the
Nursing Director.
Performs other duties as directed by the Director of Nursing or Administrator
Takes and triages all calls from nursing and home health aide staff in the field.
Takes and triages all calls from patients and their families.
Takes any clinical calls from physicians and provides complete follow through.
Enters faxed or called in physicians orders and communicates with field staff on the update.
Assesses all faxed lab values and orders and enters them into the system and notifies the clinicians in
the field.
Performs patient satisfaction calls and documents and tracks those calls weekly.
JOB CONDITIONS:
Position is stressful in terms of meeting deadlines
Primarily a desk job, which involves occasional lifting of medical supplies and charts.
On occasion, one may be required to bend, stoop, reach and move patient weight up to 250 lbs lift
and/or carry up to 30lbs.
Travel is required to and from patient homes.
The ability to communicate well, both verbally and in writing is required.
The ability to access patient's homes, which may not be routinely wheelchair accessible, is required.
Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of a
patient's condition and to perform patient care.
QUALIFICATIONS:
Must be a graduate of an accredited nursing school.
Must be a Registered Nurse licensed in the state of Massachusetts.
Must have a minimum of 2 years home health care nursing experience.
One of year management experience preferred.
Must have a criminal background check.
Must have CPR
