POSITION TITLE: LPN

FLSA CLASSIFICATION: Non Exempt

REPORTS TO: Nurse Team Lead

POSITIONS SUPERVISED: None

POSITION OVERVIEW: The LPN provides support and assistance to the Medication Management Providers and program. The LPN is responsible, in collaboration with the medication management staff, triage clients to prepare for appointments, including taking vitals, and reviewing status since last appointment, to refill prescriptions, fax and communicate lab results, and execute orders as directed by prescribing staff. The LPN will assist clients with prescription assistance plans and answer phone calls regarding medication.

ESSENTIAL POSITION RESPONSIBILITIES:

  1. Prepares patients for visits with providers by performing and recording vital signs (including blood pressure, temperature, pulse, respirations, weight, and other procedures as needed).
  2. Reviews patients' medical records for completeness and updates prior to patients' visit.
  3. Assists in preparing routine diagnostic/lab or referral request forms.
  4. Under established protocols, refills medications using the EMR, ensuring timeliness of faxes and original prescriptions as needed. Communicates with clients about the status of refills.
  5. Submits required medication documentation, in a timely manner as per agency requirements.
  6. Administers and records medication administration consistent with his/her knowledge of pharmacology in accordance with center policy
  7. Participates in preventive health teaching and education pertinent to procedures being conducted.
  8. Provides guidance to non-clinical staff regarding clinical issues (i.e. phone triage). Triages incoming and outgoing phone calls, including monitoring mental health or medication problems and reports to the physician/medical st
  9. Coordinates client appointments for laboratory tests. Informs client of laboratory results, consults with client about the medical treatment plan, documents in electronic medical record, and answers questions per the direction of medication management staff.
  10. Administers injections and other prescribed medications and treatments as ordered by providers.
  11. Demonstrates awareness of proper infection control practice by using Universal Precautions and following policies and procedures of MHA including, OSHA's Blood-borne Pathogen Standards.
  12. Routinely inspects, cleans and monitors equipment, supplies and treatment areas to ensure safety.
  13. Evaluates clients for side effects and efficacy of psychotropic medications and reports problems to the client's physician.
  14. Routinely inventories and records supply of medication samples. Destroys outdated prescriptions in accordance with Agency policy.
  15. Meets deadlines and ensures accuracy of various report/paperwork, mileage sheets, and electronic timesheets.
  16. Maintains client confidentiality.
  17. Participates in scheduled meetings and supervision with other providers.


OTHER POSITION REQUIREMENTS:
  1. Maintains acceptable overall attendance record, to include department staff meetings, agency meetings, and trainings as required. Ensures appropriate notification to supervisor for absences, and ensures that work is covered. Flexible in work schedule when needed.

POSITION REQUIREMENTS: The LPN must currently hold a license through the Kansas State Board of Nursing. Must have ability to work with diverse groups of people; particularly children and adolescents; work under pressure, & with a large degree of independence. Possess excellent verbal/written communication skills, ability to analyze, and make sound decisions. A valid Kansas driver's license and access to personal vehicle required.
