Schedule Hours for Clinic /Hospital Available for clinic questions/refills Monday-Friday 8am-5pm,seeing clinic pts approx 3-4 hrs per day, Hospital 24/7

Schedule Days Clinic /Hospital Clinic Monday-Friday 8am-5pm, Hospital 7 days per week, 24hr for calls, rounding during the day

Number of patients seen per day in clinic No more than 6

Call Night /Weekend - Yes

What is the Number of phone Consults 24/7

What are the days for Call Mon-Sunday 24/7

What Time does the Call start 24/7

What Time does Call end 24/7

What support staff is available - PA- 30 HRS PER WEEK
