Job description

We are seeking a General Dentist to cover for a maternity leave

-Dates of Coverage needed (Start date/end date)

11-6 to 1-16

-Schedule needed (days/hours):

Monday, 9-5am; Tuesday-Thursday, 7am-3pm

-Required procedures/preferred procedures:

All general dentistry, including crowns, bridges, extractions, endo preferred, some oral surgery, limited ortho

-Average number of patients per day:

24, including hygiene exams

-Average number of hygiene exams provider will perform per day:

16

-Patient demographics (% of geriatric/adults/peds)

30/50/10

-What software is used?

Dentrix

-Will provider need a Medicaid number?

No

-Insurances that provider must be credentialed with:

BCBS, Delta Dental, Aetna, AlwaysCare, Assurant, Cigna, GEHA, Guardian, Humana Metlife, Principal, United Concordia, United Healthcare

-Daily production expectation:

$4500/day

-Number of operatories in office/ number of ops that provider will work out of:

6/3

Staff 1st5r.ucture-please list number of DA's, EFDA's, RDH's, Admin, and Dr's and their level of experience:

2 RDH's, 2 DA's, 1 FO, 1 OM. Relatively new DA, seasoned RDH, currently looking to backfill RDH and DA roles.

-Any additional details that may be helpful:

We need someone who thinks comprehensively about patient care. We need someone who will look at the big picture and who will look for opportunities to do same day treatment. We prefer someone who prefers to stay busy and will

work to diagnose and fill the schedule.
