Job description
Full-time MDS Coordinator/Assistant Director of Nursing in our Long Term Care. Full-time benefits include: IPERS, Health, Dental, Vision, Life Insurance, Long Term Disability. PTO and SIck Leave Accrual begins first day of employment.

Job Title: MDS Coordinator/Assistant Director of Nursing
Primary Function and Relationship to Total Organization
Promote the physical and emotional well-being of nursing facility residents, utilizing the Resident Assessment Instrument (RAI) to gather information from residents and families during initial and periodic reviews. Formulates individualized care plans.
Provide assistance to the Director of Nursing
Reports to: Director of Nursing
Supervises: Nursing and certified nursing assistants
Essential Duties and Responsibilities
- Complete MDS assessments, initial and periodic review, assessing the emotional, mental and physical care per regulations
- Formulating and updating individualized care plans for the residents, monitoring effectiveness of the care plan.
- Schedule and conduct care conferences with residents and families
- Timely submissions of the MDS assessments according to regulation
- Conduct weekly weight meeting with dietician
- Maintaining residents vaccinations, VA eligibility, PASRR, diagnosis, DNR status, assuring accurate and up to date
- Coordinating physician visits and maintaining current progress notes
- Assisting with Quality assurance program, conducting compliance checks as directed by DON
- Assist with supervision and coaching of staff, assisting in evaluations and rounding as directed by DON
- Maintains open communication with and within Nursing staff, other departments.
- Investigates resident/family concerns and comments and takes appropriate action.
- Assists in departmental orientation of new employees and gives guidance in in-service and continuing education programs.
- Develops staffing schedule and works on nursing unit giving direct patient care.
- Keeps accurate records and logs, assuring accuracy in coding of the MDS
- Leadership on call rotation with shared 24/7 accountability with DON
- Performs functions other than described above due to extenuating circumstances

Minimum educational requirements
Current license in the State of Iowa as a Registered Nurse, Bachelor's degree in nursing preferred but not necessary, MDS Certification or agree to obtain, Successful completion of BCLS.
Experience: MDS experience preferred, 4 years of LTC experience preferred.
