Signet Health is actively interviewing Licensed Clinical Social Workers for a full-time position with our program at Franciscan Health in Michigan City, Indiana.

The Social Worker collaborates with individuals, families, and groups to address psychosocial challenges through a process of assessment, diagnosis, treatment, and evaluation. As part of a multidisciplinary care team, this position works directly with patients and their caregivers to resolve personal crises and support a therapeutic milieu to cope with chronic, acute, or terminal illnesses. The Social Worker is also responsible for overseeing the general case management and care coordination of assigned patients.

Essential Functions:

Assess the psychological status of patients related to the patient's illness and environment.
Carry out social evaluations and interventions based on evaluation findings.
Provide information to patients or families and care givers and community agencies.
Provide patient and family-focused therapeutic crisis counseling.
Continuously update case manager.
Maintain clinical records on all patients referred to social work.

Requirements/Qualifications:
Licensed Social Worker (LSW) - Indiana Licensing Board
Licensed Clinical Social Worker (LCSW) - Indiana Licensing Board
2 years Acute Care Hospital Preferred
2 years Mental Health or related Community Healthcare (preferred)
