What You'll Do


The ideal Support Services Coordinator candidate has Indiana Medicaid Waiver experience working with individuals of all ages, their families, and staff, to achieve their desired life outcomes. You will lead a growing team of Direct Support Professionals (DSPs) who serve individuals of all ages, backgrounds, and abilities in their homes and communities.

  • Manage a caseload of individuals who are supported by DSPs, including completing assessments and quarterly summaries, all while ensuring the creation of individualized outcome tracking programs that promote their desired life outcomes.
  • Work together with interdisciplinary teams which includes attending quarterly and annual meetings to discuss an individual's services and outcome optimization.
  • Supervise, support, train, and develop Direct Support Professionals.
  • Ensure services align with standards set forth by the Council of Quality and Leadership (CQL).
  • Foster and encourage natural supports for all individuals receiving services.
  • Advocate for individuals' rights and ensure they are receiving the highest standard of care.

Requirements:

Who We're Looking For

  • A bachelor's degree or equivalent experience in the human services field is preferred.
  • Experience working with the Medicaid Waiver preferred.
  • Willingness to drive to and throughout the greater Indianapolis area.
  • Willingness to work evenings and weekends as necessary to meet the needs of the individuals.
  • The ability to collaborate with local agencies and co-workers.
  • Professionalism, including the ability to build rapport, collaborate and handle difficult situations with the team and family.
  • Have an acceptable driving record and valid auto insurance coverage.
