LifeSpring Health Systems is seeking a full-time, licensed Family Nurse Practitioner or Physician Assistant to work in LifeSpring's new Community Medical Services location in Charlestown, IN. This position will work on an interdisciplinary team to provide comprehensive medical care to patients of all ages. The nurse practitioner will work to assist the patient in stabilizing any acute conditions, prioritizing care for chronic health conditions and engaging in preventative healthcare. Many of the patients seen in our practice have multiple comorbid health conditions, including behavioral health and substance use diagnos(es). The successful candidate will be comfortable practicing autonomously, will have completed a graduate program and be licensed or license-eligible in the State of Indiana. The preferred candidate will have at least three years of experience as a practitioner providing primary care to individuals with chronic conditions.
