Key Responsibilities:

As a Home Health Physical Therapist, your primary duties will include:

Delivering home-based physical therapy to patients recovering from illness or injury.
Collaborating with a multidisciplinary home health team-including RNs, LPNs HHAs, OTs, and SLPs-to ensure comprehensive patient care.
Conducting assessments, documenting progress, and adjusting care plans as needed.
Providing hands-on therapy and exercises to enhance mobility and manage pain.
Educating patients and their families on treatment plans, goals, and safe exercise techniques.

Qualifications:

To succeed in this role, you will need:

A degree from an accredited Physical Therapist program and an active PT license in Illinois.
A minimum of 2 years of physical therapy experience, preferably in home healthcare.
Current CPR and BLS certifications.
Knowledge of state and federal home health regulations.
Strong clinical judgment, excellent interpersonal skills, and a compassionate approach to care.
