gpac (Growing People & Companies ) is a national search firm that specializes in healthcare recruiting. We have been retained by a Chicago Home Health Agency to search for a Care Coordinator/Case Management Associate to join their growing team.

Position Summary: The Care Coordinator/Case Management Associate plays a critical role in coordinating and managing patient care across multiple disciplines, bridging communication between field RN Case Managers, office staff, and other healthcare providers. This role ensures continuity of care, compliance with regulatory standards, and timely support for all clinical staff. The Care Coordinator/Case Management Associate is responsible for overseeing patient progress, coordinating care plans, and ensuring that each patient receives comprehensive, high-quality care.

Key Responsibilities:

Case Management and Coordination of Care:


  • Serve as the primary liaison between field-based RN Case Managers, office personnel, and other disciplines, ensuring cohesive care plans for each patient.
  • Coordinate care plans, including admission, discharge, and transition planning, to meet patient goals and regulatory standards.
  • Facilitate communication among the RN Case Managers, physical therapists, occupational therapists, and other providers to ensure all disciplines are aligned with the patient's care plan.
  • Manage ongoing updates to patient records and ensure documentation reflects the latest care adjustments, treatment plans, and clinical observations.

Patient Progress Monitoring and Documentation:


  • Prepare and review 60-day case summaries, coordinating with RN Case Managers to discuss recertification or discharge recommendations based on patient progress.
  • Attend weekly discharge planning meetings, presenting cases for review, discussing patient goals, episode counts, and readiness for discharge.
  • Conduct follow-up communication with field RN Case Managers regarding patient needs, reassessment requirements, and changes in health status to align with treatment goals.
  • Oversee the documentation of patient care activities in the EMR system, ensuring it accurately supports medical necessity, patient status, and treatment objectives. Ensuring that visits occurred.

Scheduling and Resource Management:


  • Coordinate new patient intake scheduling, assigning disciplines based on patient needs, and ensuring timely initiation of care.
  • Work closely with therapy coordinators and other discipline managers to schedule non-nursing interventions in alignment with nursing visits and the overall care plan.
  • Assist with scheduling adjustments, confirming patient visits, and handling requests for additional services, including PRN visits, in coordination with clinical managers.

Interdisciplinary Collaboration:


  • Act as a key point of contact for all clinicians, supporting the integration of various disciplines within the patient care team.
  • Facilitate care team meetings and interdisciplinary discussions to align all providers on the care approach, address patient-specific concerns, and support the RN Case Managers in clinical decision-making.
  • Ensure smooth communication channels between field and office teams, offering clinical and logistical support as necessary to uphold quality care.

Compliance and Confidentiality:


  • Maintain strict adherence to HIPAA regulations and agency policies for patient information management, ensuring all documentation is compliant and secure.
  • Participate in audits and regulatory reviews, supporting documentation and procedural compliance with federal, state, and agency standards.

Supply and Administrative Support:


  • Manage supply requests, troubleshooting, and inventory maintenance to support field staff and patient care continuity.
  • Provide backup administrative support to the Clinical Manager/Director of Nursing as required.

Qualifications:


  • Strong case management and organizational skills, with a focus on coordinating interdisciplinary care.
  • Proven ability to support and communicate effectively with clinical staff across various disciplines.
  • Experience with case management in a home health or healthcare setting; familiarity with EMR systems (Axxess preferred).
  • Knowledge of home health care regulations and patient documentation standards.
  • Flexibility for on-call support, including evenings and weekends, as required.
