Navigators are social workers who assist Kinship families in navigating systems by linking, monitoring, and supporting through engagement in services by phone, in home and/or the community. Coordinates and assists in outreach of program services to the community and support groups. Additionally, this position is seeking candidates with strong data entry and data collection skills for this precision-focused role reporting performance outcomes and maintaining Electronic Medical Records (EMR).

ESSENTIAL FUNCTIONS:

1. Assist families in navigating systems by linking, monitoring, and supporting through engagement in services. (Level 1 families).

2. May assist FSC's in identifying resources, linkages and supports in the community or within the family's natural supports to sustain and support the family.

3. Coordinates all data collection activity necessary for funding reports and CHN quarterly reports - This position enters data into Children's Board Analytical Tracking System (CATS).

4. Maintains database and Electronic Medical Records (EMR) as defined by program standards.

5. Trains all KINSHIP team members on the use of the EMR and provides technical support to the team.

6. Establishes and maintains effective working relationships with community resource providers, school personnel, and other entities of KINSHIP.

7. Identifies challenges or issues within the program data integrity and collection.

8. Participates on projects and/or team/peer committees as available and serves as a positive representative for the program.

9. Attends trainings, seminars, conferences as directed. Shares information from training, seminars and conferences with team members.

10. Provides leadership and participates in the organization's performance and quality improvement efforts.

11. Adheres to virtual office procedures.

12. Performs other related duties as required and assigned.

MINIMUM QUALIFICATION REQUIREMENTS:

Education and Experience:

• Bachelor's Degree in Social Work, Human Services or related field from an accredited university or college required, plus two (2) years' experience in child welfare, education, and data management.

Licenses & Certifications:

• Must possess and maintain a valid Florida driver's license with no record of criminal driving offense of license suspension.

• Must possess means of providing job related transportation and show proof of required insurance. Must be insurable under CHN's current auto insurance policy.

• Must be able to work flexible hours, including evenings and weekends.

• Must successfully complete and maintain FDLE clearance, Federal background and state criminal background checks, and sexual predator screening.

• Ability to summarize in a clear concise manner care services provided to families.

• Possess skills necessary to assess, interpret and deliver care-related information concerning the age appropriate development of children and families.

• Ability to conduct assessments to plan treatment services and to provide follow-up services.

• Knowledge of intervention and behavior management methods, strategies and techniques.

• Knowledge of child abuse or neglect reporting procedures and methods.

• Knowledge of quality documentation as required by agency standards, rules, regulations and contract requirements.

• Possess a positive attitude concerning all aspects of working in a challenging environment, including significant patience and respect for children and families who can become angry and demanding.

• Sensitivity to cultural needs and willingness to serve as a positive and productive member and leader of a working team.

• Knowledge of the organization's structure, standard operating procedures and policies.

Annual Training Requirements:

• The following training topics are required annually: Sexual Harassment Prevention, Universal Precautions, Workplace Accident Prevention and Reporting, Confidentiality and Privacy Practices (Ethics), Diversity/Cultural Competency, Child Abuse and Neglect Mandated Reporter Statutes and Methods, Prevention of Violence in the Workplace, OSHA-related training, Crisis Prevention, Supervisory Techniques, and Program Trainings.

On-going Training Requirements:

• Continuing education and training as required by organization and/or program funding source.

Supervision Received:

• Weekly individual and team supervision during which time training is provided, individual families' concerns are addressed, and individual areas of professional growth are reviewed.

ESSENTIAL PHYSICAL SKILLS:

• Visual ability to read printed materials and make observations of physical facilities.

• Possess good hearing (with or without hearing devices) and the ability to understand and comprehend spoken dialogue in individual and group settings.

• Ability to use a personal computer and related office equipment. Frequent use of hands and fingers for typing.

• Ability to speak clearly and effectively.

• Ability to safely operate a motor vehicle.

ENVIRONMENTAL CONDITIONS:

• Works within an office environment, home office, or in family's home.

• High level of emotional discussions, requiring self-awareness and understanding of professional boundaries.
