Purpose and Impact:
The Case Manager is responsible for provision of comprehensive case management and support services to youth and young adults participating in Youth and Mentoring.
Essential Functions:
Youth Services
Manage refugee cases, ensuring timely delivery of services and fulfillment of requirements outlined in the Department of Children and Families Comprehensive Refugee Services contract and by LSF standards and procedures.
Provide on-going case management, assessment services, support and enrichment, career development, orientation services to refugee/entrant youth aged 16-19 for pathways to HS Diploma, GED/Alternative Programs, or Career/Employment.
Establish, facilitate and coordinate tutoring and homework assistance through program voucher system to address refugee/entrant student educational needs.
Track student progress by meeting with students individually and/or in small groups and providing resources and services as needed.
Collaborate and work closely with CRS Managers and Director and respective school, vocational program, or refugee employment in providing culturally relevant and sensitive services to clients.
Conduct curriculum-based group facilitation and activities within school settings, after school settings, and community settings as required.
Arrange and provide individual mentoring and follow up services to program participants and their families.
Mentoring Services
Create and support successful matches between mentors and mentees.
Recruit mentors, create and implement training, provide ongoing support, and program evaluation.
Oversee the Mentor Training Program to provide comprehensive training to each mentor.
Oversee program adherence to policies and procedures.
Collaborate with area schools, human service organizations, and community-based groups to advocate for families, recruit referrals, and remain up to date on data related to mentoring, youth, and family support.
Work alongside the mentors, clients, and the CRS team to plan and implement group events, discussions and activities.
Oversee data collection and maintain accurate and confidential documentation of each mentor.
Generate and submit cases' reports.
Participate in community events to disseminate program information and recruit potential clients.
Stays abreast of refugee and immigrant issues, and available services to refugees, using current information to better identify and serve refugee clients.
Represent and promotes the refugee program at meetings and speaking engagements.
Participate in trainings on cultural competence, service provision, policies and procedures.
Perform other related duties as requested by the Program Manager.
Other Functions:
Works collaboratively with all staff and school/program personnel for the purpose of accomplishing goals and objectives.
Monitor, record and maintain client file information and documentation with timely completion of charting requirements and charting accuracy. Including updates to CMS database.
Assist with program revision to reflect emerging community issues and current refugee youth culture.
Perform other duties as assigned
Provides training to program staff if necessary.
Physical Requirements:
Work is performed in an office setting at LSF, or in the community, school, vocational setting, employer throughout the SW CRS service region. Travel to off-site locations is required. The Case Manager must be able to perform the essential functions of the job. Hand eye coordination and fine manipulation skills are necessary to operate computers and office machinery. This job will call for evening and possible weekend work hours.
Education:
Bachelor's degree in Social Work, or other relevant degree or equivalent experience.
Experience:
Three years of related experience providing direct services work with adolescents and young adults between the ages of 16 - 24.
Child welfare and/or case management experience is strongly encouraged
3-year experience preferred working with refugee or immigrant children
Cross-cultural or international experience preferred
Skills:
Proficiency in Microsoft Office applications
Knowledge of community resources and ability to connect families to community services.
Excellent verbal and written communication skills
Critical assessment and analysis skills
Ability to work and deal a diverse group of clients.
Must be able to keep confidential information.
Bilingual
Other:
Must demonstrate sensitivity to our service population's cultural and socioeconomic characteristics and needs.
Principal Accountabilities:
Reports to Manager
Must be a team player with monitors as well as other clerical staff.
Accurate, complete and timely submission of required reports and documentation to DCF & LSF.
Effective working relationship with clients (providers).
Adherence to LSF policies and procedures.
