Job description

Overview

The Health Educator (CHES), in conjunction with the other team members, is responsible for the delivery of an outlined curriculum of topics which comprise the Diabetes Self-Management Education Program. This position is equally responsible for the overall curriculum for participants in individual and group outpatient settings. In addition, the CHES will provide diabetes education in an outpatient clinical setting and inpatient diabetes education when needed. The Diabetes Educator will participate with supporting and promote diabetes awareness community through participating in outreach events.
Qualifications

Minimum Education and Experience Requirements: Bachelor's degree required. Must be a graduate of approved program in health education or similar program. CHES (Certified Health Education Specialist) certification required. Preferred to have prior experience with diabetes management expertise and is either Certified Diabetes Care and Education Specialist (CDCES) or CDCES eligible. Motor Vehicle Operator Designation: Employees in this position: Will not operate vehicles for an assigned business purpose NOTE: A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job. Please indicate the appropriate operator designation on the Request for Personnel (RFP) form at the time a RFP is submitted to post the position. Licensure/Certification/Registration: CHES (Certified Health Education Specialist) certification required from the National Commission for Health Education Credentialing
