Description:

Statement of Purpose: This position is responsible for the provision of patient care under the direct responsibility of the MMU Manager and will be assigned to our mobile medical unit. Works as part of a multidisciplinary team to enhance patient quality of life. Ensures that all documentation is compliant with professional and legal standards

Essential Functions:

Front desk duties for MMU

  • Create patient accounts in eCW and make sure that new patient paperwork is complete and uploaded into chart
  • Make sure patient has up to date paperwork in chart if paperwork has expired
  • Enter in Ryan White number and coverage dates into eCW account (usually comes to you with new patient paperwork from a peer or eligibility)
  • Put in private insurance information into eCW account and email CCT to make sure that insurance is verified
  • Schedule new patient appointments in appropriate time slots
  • Schedule follow-ups for patients after they finish their appointment with Jau (ideally in person before they leave) or call patients to make sure that they are scheduled for a follow-up
  • Make sure that Ryan White is still in date prior to patient's appointment
  • Call to confirm appointments on Jau's schedule and on the MMU nursing schedule and ask patients if they need Lyft for their appointments (confirm address where patient wants to be picked up)
  • Set up Lyft's for patients the day before their appointments
  • Be available to set up Lyft's for patients to return home after appointment
  • Use BoostLingo to contact patients that do not speak English
  • Answer incoming phone calls from CCT and patients
  • Check voicemail on office phone and MMU phone daily
  • Check telephone encounters daily
  • Put location of MMU on blocks on the eCW schedule
  • Communicate via Teams with the front desk and CCT in regard to patient matters

Adheres to ethical and legal standards of practice.

Promotes and practices CAN Community Health Inc's mission and values and follows its policies and procedures.

Ensures confidentiality is maintained by entire team regarding patient/client information in accordance with HIPAA, professional and departmental standards.

Prevention Duties:

  • Perform rapid HIV, Hep C, and STI testing
  • Perform phlebotomy
  • Fill out paperwork and 1628s while testing
  • Put in results and 1628 into eCW, CTLS, and spreadsheet keeping track of numbers
  • Set up a table outside the MMU and hand out swag and information
  • Take pictures at outreach events of the team
  • Create outreach calendar and distribute
  • Create social media posts for each day we are out on the MMU and send them to Dan Furbee
  • Put outreach location and times on eCW calendar, Region 1 shared calendar, and Tockify
  • Assist in the clinic as needed
  • Drive the small outreach unit (MTU) as needed
  • Participate in outreach events outside of normal working hours
  • Participate in meetings and trainings with the prevention team
  • Network and create partnerships with community organizations and members ontinue professional training and education to advance knowledge and skills of HIV/AIDS.
  • Participates in continuous quality improvement of services rendered to CAN clients.
  • Develop and foster networking relationships with community and support resources.

Other duties:

  • Drive the MMU
  • Clean the inside of the MMU and set it up/break it down for daily use
  • Dump waste and fill up the MMU with water
  • Report issues to MMU coordinator

Knowledge, Skills and Abilities Required:

  1. Demonstrates knowledge of HIV medical terminology, procedures, medications, and accepted treatment practices.
  2. Demonstrates knowledge of OSHA rules, regulations, and mandates to ensure compliance.
  3. Good communication and interpersonal skills with patients, visitors and staff members.
  4. Ability to utilize problem-solving techniques, be a self-starter, work effectively, organize and perform multiple tasks simultaneously.
  5. Ability to read, understand, follow, and enforce safety procedures.

CAN Required Trainings:

  • General Orientation HIPAA Sexual Harassment
  • Violence in the Workplace HIV/AIDS Health Stream Courses (as assigned)

Work Environment:

  • This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, audio visual, telephones, photocopiers, filing cabinets and fax machines.

Physical Requirements:

  1. Neat, professional appearance
  2. Requires full range of body motion including handling and lifting patients.
  3. Exert up to 50 lbs. of force occasionally, and/or 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects, including the human body. Involves sitting, walking, and standing for brief periods of time.
  4. Requires expressing or exchanging ideas by means of spoken word, visual and auditory acuity.

Position Type/Expected Hours of Work:

  • This position may require additional time above normal operating hours and on occasion weekend work.

Travel:

  • This position will be assigned to staff our mobile medical unit. Travel will be primarily during the business day, although some out-of-area, weekend and overnight travel may be expected. Must be able to operate a motor vehicle and have a valid driver's license.

Education/Professional:

High School Diploma

CPR certified.

Valid driver's license.

Electronic Medical Records classes through SuccessEHS.

HIV classes through local health departments or on-line.
