Job description
CLINICAL MANAGER
Estimated pay range $75,000 - $85,000, final offer will be made based upon candidate's individual experience.
Minimum Qualifications:
1. Bachelors in Nursing preferred.
2. Current state license as a Registered Nurse.
3. Home Health experience preferred, 6 months or more of field/clinical experience
4. Excellent verbal and written communication skills and strong interpersonal skills.
5. Knowledge of home healthcare federal and state regulations.
6. Ability to utilize/learn electronic nursing documentation software.
Definition:
Provides skilled nursing care in accordance with the client's plan of treatment, including
comprehensive health and psychosocial evaluation, monitoring of services, teaching and
training activities and provision of direct nursing care. Role models the professional
workplace behavior and skills needed for high quality health care. Provides direct
supervision of Certified Nurse Aide (CNA) hiring, training, corrective actions, continuing
education, annual competencies, scheduling, work orders, and plan of care accuracy.
Level of Responsibility:
Works under the supervision of the Director of Nursing/Administrator or designee.
Exercises supervision of assigned personnel including oversight of all CNA staff and
ASSISTS with supervision of essential field staff (RN, LPN, PT, OT, ST, and MSW).
Supervisor Duties:
1. Supervises home health aides according to state regulations.
2. Responsible for ensuring timesheet entries match dates and times on visit note as
entered by the nurse. Reviews and completes staff timesheets weekly.
3. QAs all nurse notes in EMR, and addresses any discrepancies or concerns found.
4. Appropriately staffs nurse with incoming and/or existing clients.
5. Initiates nurses plan of care and ensures ongoing accuracy of all nurses care plans in coordination with the clients' primary case manager.
6. Ensures that each nurses is performing delegated tasks has a complete and up to date
Delegation form completed by the client's primary case manager.
7. Responsible for direct oversight of nurses scheduling.
8. Responsible for taking client complaints about nurses matters. Will collaborate with the Director of Nursing (DON) and Quality Assurance (QA) to ensure appropriate
investigative actions are performed and resolution is obtained.
9. Tracks nurses supervisory visits weekly, and communicates with the primary case
manager regarding any missing or non-compliant visits.
10. Performs nurses supervisory visits in the field as needed.
11. Responsible for planning and conducting monthly nurses staff meetings and training as
needed for remediation or corrective action.
12. Provides and tracks all nurses CEUs, 6 total for the year.
13. Collaborates with case managing nurses, QA, and the DON to conduct CNA
performance evaluations at 90 day and annual intervals.
Clinical Manager Duties:
1. Maintains adequate records and reports including those required by Medicare and
other third-party payers.
2. Participates in nursing in-service/training programs.
3. Participates in leadership team meetings.
4. Communicates to leadership team, PT, OT, MSW, all nursing and aide staff as
appropriate to disseminate pertinent client information.
5. Performs other duties as assigned.
6. Participates in on-call M-F after 5 pm, and all day Saturday and Sunday.
7. Performs other duties as assigned.
Field Staff Duties:
1. Initial and ongoing comprehensive assessments including Outcome and
Assessment Information Set (OASIS) for CNA only clients and occasionally other
clients per the needs of the agency.
2. Teaches, supervises, and counsels the client and family members regarding the
disease process, self-care techniques, prevention strategies, and other related
problems of the client.
3. Initiates appropriate preventive and rehabilitative nursing procedures per physician
order.
4. Supervises and demonstrates appropriate procedures as necessary to field staff.
5. Informs the physician and other personnel of changes in the client's needs.
6. Evaluates the effectiveness and outcomes of care.
7. Plans for the client's discharge from service.
8. Serves as coordinator of client's care including referral to other services as
appropriate.
9. Performs home health SN visits as needed per the agency and documents in EMR.
