Job description
Position Objective:
The Patient Care Advocate (LVN) is responsible for telephone triage and coordinating patient care once the patient has been admitted to service. This will include collaboration with the clinical team, the marketing team, patients, and their families. PCCs work closely with the clinical management team as well as the schedulers to ensure comprehensive care is implemented in a timely and efficient manner.
Job Duties
Assigns clinical team once patient is admitted to care to ensure consistency in clinical care.
Assists in fielding patient care calls to ensure staff are assigned as needed when emergent issues arise.
Assist clinical/marketing team in obtaining necessary records needed for compliance in Assisted Living and Skilled Nursing facilities.
Assist case managers in re-ordering medication as needed.
Assist/coordinate with case managers in supply ordering as needed.
Coordinate F2F visits on patients for re-certifications and manager SNF MD/NP visits as needed.
Prepare re-certification worksheets for IDG.
Run weekly stand up meetings that will include Case Managers and Scheduling team to ensure proper assignment of LVN visits.
Run compliance visit report on newly admitted patients to ensure SC, MSW compliance visits are completed within 5 days.
Schedule/prep for IDG.
Assist in facilitating IDG meetings.
Work with Medical Directors to ensure IDG coverage.
Assist DPCS/clinical manager in making welcome calls to all new admissions.
Assists Scheduling Coordinators and Intake Coordinators as needed.
Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
Coordinates the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
Establishes and maintains positive working relationships with current and potential referral sources.
Participates in quality assessment performance improvement teams and activities.
Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Mission Hospice.
Other duties as assigned by manager.
Requirements
High school graduate or equivalent required.
Licensed LVN with (1) years' experience in health care office, preferably in hospice operations.
Proficient computer skills required
Demonstrates excellence in customer service and time management
Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.
Demonstrates good communications, negotiation, and public relations skills.
Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
