Job Announcement
_ Title: Community Health Worker Division: HOPICS
FLSA: 100% Supervisor: Monnie Anderson
Pay Range: $21.00 - $24.00 per hour Revised: 08.20.24

Summary
The knowledge and skills needed for the Community Health Worker position are typically gained through a combination of lived experience with homelessness or incarceration AND navigating associated service delivery system s . For example: parole, probatio n , communit y clinics AND six months experience providing peer support and mentorship to individual s .

Essential Functions

  • Knowledge of community resources in area of residence
  • Comfortable working with diverse populations including LGBT, formerly incarcerated and mentally ill individuals
  • Exceptional ability to connect and engage with people
  • Excellent oral and written communication skills
  • Detail oriented, organized and possess time management skills
  • Able to work flexible job hours
  • Willingness to work in various environments, including jail setting s , s treet outreach, home visits, homeless encampment
  • A history of incarceration or homelessness is desirable
  • Prior experience working with currently and formerly incarcerated individuals, and/or homeless individuals is desirable
  • Prior experience as a health navigator, peer support worker, outreach worker or promotora is desirable
  • Ability to s peak Spanish , or other language s is desirable
  • Attendance in all mandatory program and division meetings and trainin g s , as assigned by his/her supervisor and division directo r .
  • Maintain appropriate boundaries; and adhere to SSG ' s Code of Ethic s and HOPICS Core Value s .
  • Represent the Agency in a professional manner at meetings and community events.
  • Maintain files/records on client services in compliance with HIPAA, 42 CFR Part 2 and other funding requirements for audit purpose s .

Minimum Qualifications - Knowledge, Skills and Abilities Required
Valid CA Class C Driver License or access to reliable transportation to carry out jo b - related essential functions . High school diploma or GE D . Bac h e lo r 's Degree from an accredited college/university desire d . Proficiency in Microsoft Wor d , E x ce l , and Interne t . If in recover y , a minimum of three (3 years of being drug and alcohol free is require d . Neat and prof e ss i onal appe a r ance. Attention to detail and punctuality are require d ; Ability to communicate effectivel y , bot h written and orally require d . TB test, CPR and First Aid certification require d . Note: Results may not be more than 3 months prior to or 7 days after date of hire and renewed annually thereafter.)

Supervisory Responsibilities
This position does not have any supervisory responsibilitie s .

Environmental Conditions (Working Conditions)
This position is responsible for working in "client friendly" environments and may be required to visit other shelters and partner agencie s . There is some responsibility to work in noisy environments where children and unstable adults are few to talk loud and expressive.

Physical Requirements
The staff typically spends time sitting, standin g , walkin g , readin g , thinkin g , note takin g , and driving.

Mental Requirements
This position will require the individua l to be able to handle any/all the following: constant distraction s , interruptions, uncontrollable changes in priorities/work schedule s : to be able to process informatio n , thin k and conceptualiz e .
