Overview POSITION SUMMARY This position is responsible for providing diabetes self-management education/training to patients diagnosed with diabetes, using evidence-based and public health approaches to treat and prevent diabetes and its complications. The incumbent's duties also consist in the coordination and delivery of group education classes as available, along with a systematic approach to case managing diabetes care of both adult and pediatric patients. Qualifications NECESSARY QUALIFICATIONS Education: Bachelor's degree in Nursing Certifications: Must have a valid, current, full and unrestricted Nursing license to practice nursing in any state of the U.S. commonwealth of Puerto Rico or U.S. territory Must have and maintain current Basic Life Support (BLS) certification by the American Heart Association (AHA) Experience: Must have at least two (2) years of experience as a Registered Nurse, preferably with type 2 diabetes mellitus Other Skills and Abilities: A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in each one of the following areas: Positive working relationships with others; cooperating effectively in teams for the benefit of patient care Possession of high ethical standards and no history of complaints Reliable and dependable; reports to work as scheduled without excessive absences. Effective verbal and written communication skills; exceptional communication and interpersonal skills are essential. Ability to prioritize duties and work independently; must be self-motivated. Experience with Microsoft Office and other computer applications. MENTAL & PHYSICAL EFFORT The physical and mental demands described here are representative of those that must be met by an individual to successfully perform the essential functions of this job. PHYSICAL Incumbent must be able to sit at a desk for long periods of time, write legibly, and use a computer terminal. The incumbent may need to frequently stand and walk; and occasionally need to drive, bend, climb, kneel, crouch, twist, maintain balance, and reach. This position may need to push, pull, and lift up to 25 pounds occasionally. This position is primarily a sedentary position in an environmentally controlled area. The incumbent must be able to able to hear normal speech, hear overhead pages on a loudspeaker; and talk over the telephone and in person with others for long periods through the workday. Other sensory requirements required is frequent use of far vision, near vision, and seeing in fine detail. The incumbent may occasionally need to be able to have color vision and depth perception. This position requires the ability to use a keyboard for long periods throughout the workday. May also need to occasionally be able to use hand manipulation of simple and firm grasping, and fine manipulation. MENTAL The incumbent must be able to prioritize and use good judgment; must be able to coordinate a variety of issues while being frequently interrupted; and must be able to assess data such as lab values and weights. This position requires the incumbent to handle a high degree of flexibility and demonstrate a high degree of patience for more than of the workday. More than the workday the Incumbent must be able to concentrate, handle multiple priorities in stressful situations; work alone; and work in areas that are close and crowded. The incumbent may occasionally need the ability to cope with high levels of stress, make decisions under high pressure, cope with anger/fear/hostility of others in a calm way, manage altercations, and adapt to shift work. ENVIRONMENTAL: This position will occasionally work around infectious diseases; chemical agents; dust, fumes, gases; extremes in temperature and humidity; and hazardous or moving equipment. Must be able to occasionally accept a flexible schedule to meet unit needs. Responsibilities ESSENTIAL FUNCTIONS Provides inpatient and outpatient diabetes self-management education, including but not limited to diabetes pathophysiology, acute and chronic complications, basic nutritional management, psychological adjustment, medications (including insulin), physical activity, monitoring, and goal setting. Performs individual patient needs assessment to evaluate intervention priorities. Assesses patient needs, medical condition, lab values, nutrition and weight status, self-management abilities, psychosocial concerns, learning factors, mental and emotional health status, access to health care resources, and glycemic control. Provides appropriate level and frequency of care for patients and their families as identified in the assessment. Provides diabetes self-management education in both individual and group settings utilizing the IHS "Balancing Your Life with Diabetes" and Association of Diabetes Care and Education Specialist curricula. Provides and documents services in such a way as to ensure maximum reimbursement by third party payers. Understands and applies the American Diabetes Association and IHS Diabetes Standards of Care. Applies theoretical approaches such as motivational interviewing, stages of change theory, and empowerment theory to facilitate behavior change. Effectively communicates with patients and families in a culturally sensitive manner. Recommends adjustment of treatment plan utilizing patient blood glucose monitoring data to optimize glycemic control. If adjustments in medications are indicated, consultation with the primary care provider or attending physician is done; if the primary care provider or attending physician is not available, consultation will be done with the physician on call from the same service. Medication management may be ordered by the provider treating the patient's diabetes and any adjustments require a verbal order from a provider as above. Utilizes a case management model to follow referred patients. Ensures patients receive recommended services and appropriate follow-up per current clinical practice guidelines. Develops patient education materials as needed, seeking approval for use of new or updated materials through the Patient Education Materials Committee. Maintains current licensure, job-related competencies, and knowledge of evidence-based guidelines through active participation in continuing education, independent study, journals, seminars, in-services, and dialogue with colleagues. Develops and assists with Performance Improvement initiatives. Understands and adheres to established compliance, safety, and quality assurance standards. Employs the Association of Diabetes Care and Education Specialists ADCES7 Self-Care Behaviors framework. Ensure Proper PPE is always used while on duty. Face surgical mask is to be worn in all hospital areas. Proper handwashing is required. If water is not available use hand sanitizer. Social distancing will be practiced by department. Proper PPE is used inside assigned Clinics or Unit. NIOSH-approved N95 mask filtering face piece respirator or higher, is available, this included eye or face masks shield, gloves, and isolation gown. Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. Complete provided training. Performs other duties as assigned
