Position Overview:
We are seeking a skilled and compassionate Occupational Therapist (OT) to join our home health care team. The OT will provide home visits to patients in the Sedona area, helping them regain independence and improve their quality of life through customized therapeutic interventions. The ideal candidate will live in or around the Sedona or Cottonwood areas and be willing to commute to the Flagstaff or Prescott regions as needed.

Key Responsibilities:
Provide in-home occupational therapy assessments and develop personalized treatment plans.
Help patients enhance their daily living skills and functional abilities to promote independence.
Collaborate with the interdisciplinary healthcare team to ensure coordinated care for each patient.
Educate patients and their families on adaptive equipment, home modifications, and therapeutic exercises.
Maintain accurate and up-to-date documentation of all therapy sessions, assessments, and patient progress.
Participate in case conferences with other home health team members to discuss patient care and improvements.

Qualifications:
Bachelor's or Master's Degree in Occupational Therapy from an accredited program.
Current licensure to practice as an Occupational Therapist in the state of Arizona.
Minimum of 1-year experience as an Occupational Therapist, preferably in a home health or healthcare setting.
Strong knowledge of therapeutic techniques, adaptive equipment, and patient-centered care.
Excellent communication, organizational, and time-management skills.
Willingness to commute to the Flagstaff or Prescott areas as necessary.
