Position Profile The Licensed Practical Nurse (LPN) will support Geriatric Solutions, a program of Hospice of the Valley, which provides primary medical care for home-limited individuals in the comfort of their home, wherever they live. This position will provide care coordination and serves as liaison between patients, families, interdisciplinary team, referral sources and other community providers to facilitate care. This role will triage incoming phone calls and perform follow-up and/or communicate with provider or other practice staff to meet patient needs. This role includes making home visits and/or provide hands-on support as needed. Responsibilities + Develops and maintains therapeutic relationships. + Provides accurate observation of patient needs. + Provides quality nursing care. + Provides effective pain/symptom management. + Promotes patient safety. + Provides effective patient/family/caregiver teaching. + Triage incoming phone calls and performs follow-up and/or communicates with provider or other practice staff to meet patient needs. + Coordinates prescription orders and refill requests with pharmacy. + Perform home visits and/or provide hands on support as needed which may include, but is not limited to: vital signs, phlebotomy, vaccinations, procedure assistance and patient positioning as requested by the provider. + Provides insight and guidance on standardizing referral process for community resources and assist with establishing and maintaining relationships with partner agencies such as home health, hospice, social work and senior services. + Creates timely and accurate documentation. + Utilizes resources effectively and efficiently. + Adheres to scope of practice/HOV standards and facilitates continuously improved processes/services. + Maintains and enhances professional skills. + Adheres to high standards of personal and professional conduct. Minimum Qualifications + Graduate of accredited school of nursing. + Current Arizona LPN licensure in good standing. + Minimum 1 years of experience in primary care, palliative or hospice care. + Current CPR certification + Valid Arizona driver's license and insured automobile. + Excellent communication and interpersonal skills. + Strong clinical assessment skills. + Solution driven, creative and resourceful problem-solving skills. + Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through. + Proficient computer skills. Preferred Qualifications + CHLPN certification.
