JOB SUMMARY:
The Home & Community Based Service Care Provider will provide specialized sensory-motor, cognitive, communicative, social interaction, and behavioral training to promote skill development in individuals who have developmental disabilities by providing one or more of the following services:
RESPITE - ESSENTIAL FUNCTIONS
1. Provide relief to a family member or person caring for a consumer.
2. Provide supervision either in the home or community and support the emotional, physical, and mental well-being of the consumer.
3. Determine the consumer's routine plan of care from the consumer's caregiver.
4. Ensure that the consumer receives medications as prescribed.
5. Provide first aid and appropriate attention to injury and illness.
6. Ensure provision of food to meet daily dietary needs. Therapeutic diets requiring specialized ingredients or food supplements will be supplied by the family.
7. Assist the consumer in utilizing transportation to support the consumer in all daily living activities.
8. Carry out any programs identified in the consumer's ISP and/or routine plan of care.
8. Provide observation of client's behavior, immediately reporting any potentially dangerous situations to the responsible person and supervisor.
9. Schedule services and appointments to coincide with family preferences and routines.
10. Maintain accurate documentation of all required paperwork and submit timely.
HABILITATION - ESSENTIAL FUNCTIONS
1. Enable the consumer to acquire knowledge and skills and be a valued member of his/her community based on his/her own choices.
2. Provide training to increase or maintain the consumer's self-help, socialization, and adaptive skills.
3. Assist the consumer in achieving and maintaining a quality of life that promotes the consumer's vision of the future.
4. Provide first aid and appropriate attention to injury and illness.
5. Carry out any programs identified in the consumer's ISP and/or routine plan of care.
6. Provide observation of client's behavior, immediately reporting any potentially dangerous situations to the responsible person and supervisor.
7. Schedule services and appointments to coincide with family preferences and routines.
8. Maintain accurate documentation of all required paperwork and submit timely.
ATTENDANT CARE - ESSENTIAL FUNCTIONS
1. Implement the general plan of care based on the ISP-approved Attendant Care Assessment.
2. Provide assistance to maintain personal cleanliness and in activities of daily living that do not require medical supervision or intervention. Tasks may include but are not limited to: bathing, oral hygiene, toileting, bowel and bladder care, dressing, ambulation, transferring to and from wheelchair and/or bed, eating and meal preparation, routine nail and skin care, tasks necessary for comfort and safety of movement-restricted consumers, and assisting with special appliances and/or prosthetic devices.
3. Provide assistance by planning, shopping, storing, and cooking food for nutritional meals.
4. Ensure that the consumer receives medications as prescribed.
5. Provide first aid and appropriate attention to injury and illness.
6. Provide assistance to attain or maintain safe and sanitary living conditions. Tasks may include but are not limited to: dusting, cleaning floors, cleaning the kitchen, washing dishes, changing linens and making bed, washing, drying, and folding laundry, shopping for and storing household supplies and medicines for the consumer, taking out the garbage, and other duties as determined appropriate and necessary by the consumer's ISP team.
7. Provide observation of consumer's behavior, immediately reporting any potentially dangerous situations to the responsible person and supervisor.
8. Schedule services and appointments to coincide with family preferences and routines.
9. Maintain accurate documentation of all required paperwork and submit timely.
Requirements
GENERAL RESPONSIBILITIES
1. Schedule all appointments and visits to coincide with family preference and routine.
2. Ability to work weekends and evenings as required by the needs of each consumer/family.
3. Maintain professionalism at all times while representing Family Partners, LLC.
4. Provide safe transportation to and from the home and community activities.
PHYSICAL/MENTAL DEMANDS, ENVIRONMENT
1. Possess organizational skills and the ability to multi-task.
2. Must have outstanding communication and social skills.
3. The ability to initiate tasks necessary for the program to run efficiently.
4. The ability to work well under pressure.
5. Ability to problem solve.
MINIMUM QUALIFICATIONS:
To function effectively in this position, the incumbent must possess or obtain:
• High School Diploma or G.E.D.
• Valid Arizona Driver's License
• Reliable means of transportation with current insurance and registration
• Ability to obtain an Arizona Fingerprint Clearance Card
• Ability to pass criminal, background and reference checks
To function effectively in this position, the incumbent must also pass the following trainings provided by Family Partners:
• Article Nine Training
• Certification in Adult/Child/Infant CPR, and First Aid
